What can be detected in prostatic fluid?

Generally, when you go to the hospital for a checkup, it is mainly divided into different types of symptoms. Prostate diseases are relatively common nowadays, and many male friends go to the hospital to check this problem. During the examination, they will encounter a prostate fluid test, which is mainly through testing and detecting the patient's physical condition, and then giving a standardized diagnosis and reasonable treatment.

Clinical significance

Abnormal results: When prostate cancer grows within the capsule, it is difficult to diagnose by CT.

It can only be diagnosed when the prostate has infiltrated into the surrounding fat tissue. The manifestations are irregular prostate contours and uneven density. The anterior rectal wall and bladder wall may be infiltrated, and the seminal vesicle angle disappears. CT can also detect lymph node metastasis and pelvic metastasis. CT is helpful for staging prostate cancer.

On MRI, prostate hyperplasia mainly occurs in the central area, so T2WI shows enlargement of the central area. Prostate cancer mostly occurs in the peripheral area, resulting in a limited low signal or slightly low signal nodular shadow in the peripheral area on T2WI. Therefore, MRI is more valuable in diagnosis than CT. However, when the MRI manifestations of prostate cancer are atypical, diagnosis is still difficult.

How to diagnose

1. Most patients take the knee-chest position or lithotomy position. If the patient is seriously ill or weak, the side-lying position can also be taken.

2. The doctor wears gloves or finger cots and applies vaseline or liquid wax on the fingertips.

3. In the knee-chest position, support the patient's left shoulder or buttocks with your left hand, and gently massage the anus with your right index finger to help the patient adapt to the massage, so as to avoid sudden tension of the anal sphincter. Then slowly insert your finger into the anus, and when the fingertip enters the anus wall about 5 cm away from the anus, you can touch the prostate, and pay attention to the shape and changes of the prostate.

4. When massaging the prostate, use the distal ends of your fingers to slowly massage inward and forward, about 4-5 times on each side. Then move your hands to the upper part of the gland and squeeze downward along the median groove. This will allow the prostate fluid to be discharged from the urethra and a sample can be collected for testing.

Notes

1. The indications for prostate massage should be clear. It is generally used for chronic prostatitis. Massage is contraindicated if tuberculosis, abscess or tumor is suspected.

2. The force used during massage should be even and appropriate. If it is too light, the prostate fluid cannot be expelled, and if it is too heavy, it will cause pain.

3. Massage should be performed in a certain direction and should not be performed back and forth. Unreasonable techniques often lead to failure of the examination.

4. If a massage fails or the test result is negative, it needs to be repeated after 3-5 days if there are clinical indications.
