1. Classification of etiology and pathogenesis

More complicated, the pathogenesis may include the following factors:

(I) Relative or absolute increase in estrogen

Testosterone can be converted into estrogen in the body. For particularly sensitive breast tissue, testosterone itself can also cause hyperplasia.

(ii) Increased pituitary gonadotropin

In particular, the luteinizing hormone, also known as prolactin, can cause the production of connective tissue between the mammary ducts and increase adipose tissue.

(iii) Increased sensitivity of breast tissue to the trace amounts of estrogen normally found in male body fluids.

(iv) Use of estrogen or drugs with estrogen-like effects such as reserpine, isoniazid, and human gonadotropin.

The cause of this disease is complex. A correct diagnosis is made and then treatment is carried out according to the cause. Primary male breast hypertrophy in adolescence often tends to heal on its own and can return to normal within about 6 months. Primary male breast hypertrophy in adults and the elderly can partially heal on their own, while others require medication. As for secondary male breast hypertrophy, after a clear diagnosis, treatment is carried out according to its cause. When the primary lesion is cured, the enlarged breasts can often gradually recover.

2. Drug treatment

(1) Selective estrogen receptor antagonists such as tamoxifen.

(2) Androgen therapy can reduce breast development in patients with androgen deficiency, but it is not recommended for patients with normal androgen levels because androgens are often converted into estrogen in the body and aggravate breast development.

(3) Aromatase inhibitors such as testosterone can inhibit the aromatization of androgens in the body, thereby reducing the production of estrogen.

In addition to etiological treatment and drug treatment, surgical treatment can be used to treat male breast hypertrophy.

(1) Indications: ① Male breasts with a diameter greater than 4 cm that cannot be reduced for a long time; ② Those whose breast enlargement significantly affects their appearance; ③ Those who have no significant effect after regular medication treatment; ④ Those who suspect that the disease has worsened and are psychologically nervous and uneasy.

(2) Surgical method

① Subcutaneous mastectomy (nipple and areola reserved) is suitable for young patients; ② Simple mastectomy (without nipple and areola reserved) is mostly suitable for elderly patients. The specimen removed during surgery is sent for pathological biopsy for confirmation.
