The Chinese translation is based on the actual condition, and is sometimes also called abnormal penile erection or painful penile erection.

In the past, it was believed that the mechanism of sustained penile erection was due to blood stasis caused by obstruction of the venous outflow tract of the corpus cavernosum of the penis. When blood stagnates in the corpus cavernosum, the viscosity of the blood increases, the oxygen content in the blood decreases, the pH decreases and becomes acidic, and the carbon dioxide content increases, further causing edema of the interstitial space between the sinuses, ultimately leading to venous embolism, blood coagulation, and blood clot organization, which causes the corpus cavernosum to become fibrotic, and the corpus cavernosum becomes a solid cord that can no longer be filled with blood and erect.

With the deepening of scientific research in recent years, the principle of sustained erection has been understood more clearly. Sustained erection of the penis can be divided into two types, namely high-flow type (non-ischemic type) and low-flow type (ischemic type). The former is due to excessive blood inflow into the artery, but the venous outflow is not blocked. At this time, the smooth muscle of the cavernous sinus is not relaxed, so the excess blood inflow only causes partial dilation of the sinusoidal space, and most of the blood flows directly through the uninvolved subalbuminae venous clusters. Therefore, there is no stagnation and ischemia of the blood in the cavernous sinus, so generally no pain is caused. Low-flow persistent erection is mainly caused by obstruction of the cavernous vein. At this time, the arterial blood supply is also reduced, so tissue hypoxia and acidosis develop rapidly, which can easily cause smooth muscle paralysis in the cavernous body and small artery walls, resulting in thrombus formation in the cavernous body cavity, causing complete closure of the venous output duct, and the extremely high pressure in the cavernous body causes the arterial input to stop completely. Due to ischemia and hypoxia of the cavernous body, severe pain can occur.

Regardless of the type of sustained penis erection, although it appears that the penis is erect for a long time, a closer look shows that only the two corpora cavernosa on the back of the penis are filled, and the corpora cavernosa of the urethra is not filled, so the glans penis is also soft. The difference between the two types is that the skin color and elasticity of the high-flow type penis are still good, generally there is no pain, and sometimes the erection lasts for several days and sexual function can be maintained after recovery. The low-flow type has a bluish-gray skin due to severe ischemia, and the corpus cavernosum is hard as wood. There is also severe pain and difficulty walking. If not treated in time, it will quickly lead to necrosis and fibrosis of the trabecular structure of the corpus cavernosum. Therefore, it should be treated as an emergency to avoid the sequelae of permanent impotence.

The main reason for persistent erection should be found and targeted treatment should be carried out. If the cause is not easy to find at once, it should also be treated in time. In the early stage, vasoconstrictors such as Alamin can be injected into the cavernous body to shrink the cavernous sinus and quickly discharge the blood. If the injection cannot solve the problem, a thicker needle should be used to puncture and draw blood, and then irrigate with liquid containing vasoconstrictors to make the penis flaccid. Finally, if there is really no way to solve it, the only option is blood shunt surgery, but you should be mentally prepared for the possibility of impotence after the surgery.

Therefore, men should reduce stress appropriately in their daily lives and try not to carry too much psychological burden.
