There is a circle of small rashes around the edge of the glans

The main reason for the rash is due to fungal infection or the skin being in a humid environment for a long time. Therefore, everyone should pay more attention to personal hygiene in order to avoid the occurrence of rash problems. Male penis infection with fungi may cause pearly rash. This disease will not affect the individual's fertility, but there will be a foreign body sensation, which will affect the individual's life to a certain extent.

It is a benign penile pearly rash.

Penile pearly rash is a physiological developmental variation that does not cause functional disorders. Clinically, it often occurs after puberty.

According to foreign reports, the incidence rate in normal adult males is about 10%, and domestic reports are 24%. Clinically, it is difficult to distinguish from early atypical condyloma acuminatum and is easily misdiagnosed. It is large in size and has obvious keratinization, and is easily misdiagnosed as sebaceous gland dysplasia. The reasons for the misdiagnosis of this disease are as follows:

① In recent years, the incidence of sexually transmitted diseases has been on the rise. Clinicians are more vigilant about genital warts. When encountering proliferative lesions on the glans penis, genital warts are often considered first. In particular, some patients have a history of sexual intercourse or are combined with other sexually transmitted diseases, which makes it easier to misdiagnose them as genital warts. This is the main cause of misdiagnosis.

②Pearlitis of the penis is a physiological developmental variation, and the clinical manifestations of this disease are non-specific. Doctors from other departments lack specific auxiliary diagnostic methods, and the examination time is insufficient and the examination is not meticulous, which makes it very easy to make misdiagnoses. In this group, 8 cases were referred from the infectious disease department and the surgery department, reflecting the non-standardization of STD diagnosis and treatment in primary hospitals.

③ After discovering skin lesions, patients mistake them for condyloma acuminatum, and they panic and scratch repeatedly, causing the rash to redden and break, which makes clinical diagnosis difficult. For some papules that are larger in size and have obvious keratinization, it is difficult to distinguish them from sebaceous gland dysplasia clinically. Once this disease is misdiagnosed as condyloma acuminatum, it not only causes mental burden to the patient, but also affects the marital relationship.

Through clinical observation and analysis, we have realized that in order to avoid or reduce misdiagnosis, we should pay attention to the following points:

①Penile pearly rash is not contagious, does not cause functional disorders, and does not require any treatment. However, condyloma acuminatum is one of the most common clinical sexually transmitted diseases. It is obviously contagious and extremely harmful to health, so it should be treated promptly.

②Penile pearly rash is mainly confined to the dorsal side of the coronal groove, and is a small yellowish or reddish papule with a smooth surface like caviar; although condyloma acuminatum is a small papule in the early stage, it is often pointed, with a height greater than the horizontal diameter, and the distribution is mostly asymmetrical. Later, it often increases and fuses into a cauliflower shape, which is prone to erosion and bacterial infection.

③ Pathological sections of condylomas show mild hyperkeratosis of the epidermis, thickening of the stratum spinosum, papilloma-like hyperplasia, and characteristic vacuolated cells in the epidermis. Apply 5% acetic acid solution soaked in gauze or cotton balls to the skin lesions for a few minutes and then remove. The penile pearly rash does not change color (negative reaction); the rash of condylomas mostly turns grayish white (positive reaction). The histological manifestation of sebaceous gland ectopy is mild hyperplasia of the epidermis and more mature sebaceous glands in the dermis.

④ For cases that are difficult to diagnose at the moment, a pathological biopsy should be performed as soon as possible. If the patient does not agree to the biopsy, no diagnosis will be made for the time being and regular follow-up examinations will be conducted. Most penile pearly rashes do not change significantly for a long time, while the damage of condyloma acuminata can increase rapidly over a certain period of time.
