Balanitis can infect the urinary and reproductive system in reverse, causing cystitis, pyelonephritis, etc. In addition, if the inflammation cannot be cured for a long time, it can directly affect the sexual life of the couple, and then cause impotence, premature ejaculation, etc. The disease is more common in autumn and winter, and mostly occurs in young men, especially those with long foreskin or prepuce. Symptoms; 1. Subacute atrophic glansitis It is common in young people with active sexual life. The infection bacteria are generally the same as those of the sexual partners. At the beginning of the disease, part of the skin is red, and the active glans penis has a burning and itching sensation. Acute symptoms: When the foreskin is opened, the glans penis can be seen to have edematous erythema, erosion, exudate and bleeding. In severe cases, blisters may appear. If secondary infection occurs, ulcers may occur, covered with purulent exudate, and some pain and tenderness are obvious. Some patients may have mild systemic symptoms, such as fatigue and drowsiness. During the onset of the disease, only the inner plate of the foreskin and the coronal sulcus are red and blocky, or cracked like broken porcelain. 2. Annular rupture of glans It may be the initial mucosal damage of Reites disease, or it may be related to the long-term irritation of urine scale. The clinical symptoms are erythema on the glans penis, which gradually expands and becomes annular or multi-annular, and then shallow ulcers may occur. The key feature of the disease is one or more annular lesions on the glans penis, with a bright red erosive surface in the middle and a white thin band on the edge, connected into a ring. If secondary infection occurs, the symptoms may be aggravated and lose its annular characteristics. 3. Trichomonas balanoposthitis Caused by trichomonas infection, Candida albicans is the most common. Clinical symptoms include prepuce hyperplasia and erythema of the glans penis, smooth surface, a small amount of scaling nearby, scattered papules or pustules around, and slowly expanding to the periphery. Acute symptoms may present with edema erythema, erosion, and exudate. In severe cases, it may spread to the male genitals, scrotum, thigh and groin. It is mostly caused by sexual transmission, and can also be secondary to diabetes, active hepatitis, and long-term antibiotic or hormone treatment. 4. Cytoplasmic balanitis It is more common in middle-aged and elderly patients. Generally, there are no significant active symptoms. The glans penis produces tolerant, localized, infiltrative, dark red and dark brown patches with clear boundaries, smooth surface, many red spots or erosions, easy bleeding, and no ulcers. Histopathology has diagnostic value, and its characteristics are that there are band-like inflammatory cells infiltrating in the superficial dermis, mixed with many plasma cells. 5. Amoebic operative balanitis When the glans penis becomes inflamed due to prepuce hyperplasia, the natural barrier function of the epithelial cells is lost, which can easily lead to amoeba infection. The clinical symptoms are infiltrated erythema, erosion, shallow ulceration, significant tissue necrosis and pain on the glans penis. 6. Biotitic and keratinizing pseudoepitheliomatous glansitis It is common in patients over 50 years old and is often accompanied by prepuce. The skin of the glans penis is infiltrated and hypertrophic, with excessive keratinization in some parts, and silvery biotite crusts. The glans penis loses its normal elasticity and gradually shrinks. 7. Candidal balanitis It is a mild transient erosive glans inflammation caused by trichomonas infection. Patients often experience itching, and the symptoms worsen when urinating. Initially, erythema and papules appear on the glans, with clear boundaries, and the range gradually expands. There will be small blisters ranging from the size of a needle to a millet on the erythema, which can expand and merge with each other, and produce a slightly erosive surface. 8. Dry obstructive glans In the early stage of the disease, there is chronic glans inflammation, hypertrophy of the mucosa, and the affected area is white like a hippopotamus tooth, and the outer skin may be flaky. In the late stage, the part is shrunken like a scar. It can cause the external urethral opening to be narrowed and the foreskin to adhere. |
