Abnormal sperm morphology often exists at the same time as oligospermia or poor vitality, but sometimes it exists alone. The evaluation of sperm morphology is based on the entire sperm: the head, midpiece and tail. The normal range of sperm morphology and size is: (1) Head: The head is 3.5-5.0 microns long and 2.0-3.0 microns wide; the sperm head is a regular oval. When the length and width of the sperm head exceed 5 and 3 microns respectively, it is called a large head; when it is less than 3 and 2 microns respectively, it is called a small head; if the size is unclear or the shape is close to an oval, it should be counted as normal, because it has been found in practice that as long as other morphological parameters of the sperm are normal, the size of the sperm head is not an important factor. Sperm head abnormalities with clinical significance include: conical (length and width are less than 5 and 2 microns respectively, or longer than 5 microns and less than 3 microns wide), irregular, double-headed and "pear-shaped". Among them, the "pear-shaped" head is closely related to infertility in cattle, and its impact on human fertility has not been further introduced. It is generally counted as irregular or conical. If the judgment is unclear, it will still be calculated as normal. (2) Midsection: The midsection is 5.0-7.0 microns long and 1.0 microns wide. A width of more than 2 microns usually reflects that the cytoplasmic droplets have not completely degenerated, and its impact on infertility is still unclear. If the cytoplasmic droplets are larger than half of the sperm head, it means that the sperm is immature and is abnormal. If it is less than or equal to half of the head, it should be normal.
