The injury to the corpus cavernosum of the penis cannot heal itself, because the corpus cavernosum of the penis is a relatively sensitive part and is easily stimulated. Stimulation will cause the penis to become erect, and if the corpus cavernosum is damaged, it will cause local swelling and congestion of the penis, and may also cause erectile dysfunction. If the corpus cavernosum of the penis is damaged, the penis should be prevented from being stimulated, especially from being squeezed. The damage to the corpus cavernosum depends on the severity of the damage. If it is a mild damage with local swelling and congestion but no obvious necrosis, it can certainly heal on its own. Of course, if the corpus cavernosum is slightly damaged, it may also heal on its own. The corpus cavernosum is divided into the penile corpus cavernosum and the urethral corpus cavernosum. If the urethral corpus cavernosum is damaged and the urethra is involved, recovery will be relatively complicated. If the corpus cavernosum of the penis is simply damaged, it will be easier to recover because it is all muscle tissue. However, the premise is that there is no nerve damage. If there is damage to the penile nerve, dorsal nerve, or nerves in other parts, it will be very difficult to recover. Because of nerve damage, the self-recovery ability of the nerve is relatively poor. If it is not disconnected, it may recover. If it is disconnected, it will be difficult to recover, and it needs to be evaluated according to the damage. Most injuries are relatively minor, unless they are violently squeezed or cut. How to restore the damaged cavernous body Damage to the corpus cavernosum is usually caused by external forces after the penis is erect, which will cause hematoma and is very painful. It seems that your corpus cavernosum is not damaged, but rather the penis is bent or lateral. Under the influence of frequent external forces such as masturbation and certain working postures, the entire corpus cavernosum is translated inside and outside the body. In other words, the entire penis strip is deflected in the abdominal cavity, which can be corrected through regular massage. The treatment of cavernous body injury includes analgesia, debridement under local anesthesia, suturing, reduction, etc. In clinical practice, not all injuries will affect sexual function. The other is a congenital curvature, which is usually not easy to find when the penis is flaccid. When the penis is sexually excited, it will bend to the left or right, upward or downward, etc. The arc shape can usually be seen from the outside of the penis. Suggestions: This phenomenon is more common in Western countries such as Europe, and is most common in the form of upward curvature after sexual erection. This is related to racial genetics, but it is not a pathological manifestation. Generally, if it is estimated that it will not affect future sexual life, it does not need to be corrected. If the curvature is too large, it needs to be corrected through surgery, but it may affect the quality of the cavernous body erection after the operation. Traumatic injury to the corpus cavernosum of the penis can easily cause penile curvature after erection, which can lead to difficulty in sexual intercourse in severe cases. Experts report that 5% to 15% of patients experience impotence after penile corpus cavernosum injury, mainly due to penile curvature and erectile pain. When treating penile injuries, it is important to protect blood vessels and nerves to reduce the occurrence of impotence after injury. Cavernous body injury is related to many factors, but the vast majority of the causes are trauma. If we can avoid trauma, there may not be much damage. We must protect the cavernous body, because cavernous body injury may lead to male sexual dysfunction. Types of cavernous body injuries include 1. Compression injury The penis is squeezed by heavy objects for a long time, causing edema and necrosis of the penis. It often occurs when trenches, tunnels, and houses collapse. 2. Impact Injury Due to careless work and other reasons, the penis is hit and local congestion and swelling occur. 3. Stab The wound is often small and deep, with poor drainage and prone to deep infection. 4. Gunshot wounds When shrapnel enters the wound, it often brings in foreign objects such as soil, cloth, and cotton, and the wound is often contaminated. 5. Cutting injuries The wound edges are mostly neat, but there is a lot of bleeding. 6. Bites Bites from animals or humans can easily lead to infection. As for the various types of cavernous body injuries introduced above, we can avoid being injured by them as long as we try not to touch them. Cavernous body injuries generally have the following symptoms. If we have these symptoms, we must pay attention. Symptoms of cavernous body injury The corpus cavernosum of the penis has abundant venous sinuses. There are three corpora cavernosa in the penis. These three corpora cavernosa are wrapped by fascia and have abundant blood vessels and nerves running inside the penis. Therefore, penile injuries can present very prominent clinical symptoms. The prominent manifestations of penile injury are local pain, as well as swelling, bruising, bleeding, laceration, transection, penetration, dislocation, exfoliation, necrosis, and partial defect. How to check for cavernous body damage (1) Pharmacological cavernography After the iodine allergy test, the penile skin is routinely disinfected. After applying pressure to the base of the penis with a rubber band, a 7-gauge needle is used to inject vasoactive substances, such as castor oil and prostaglandin E1, into the posterior 1/3 of the penis. After observing the penis erection for 15 minutes, 80 ml of 30% diatrizoate is selected and injected directly into the corpus cavernosum on one side. After satisfactory imaging, anteroposterior and left and right oblique films are taken. (2) Continuous infusion angiography After the injection of vasoactive substances into the penile cavernous body, when the erection is below 60 degrees or disappears quickly and the penile arterial system is checked within the normal range, a Swiss Gombro perfusion pump is used to automatically control the liquid flow rate per minute. The patient lies supinely and routine disinfection is performed. One end of the catheter connected to the perfusion pump is connected to a No. 9 needle and inserted into the cavernous body of the penis on one side. The other end of the catheter is connected to an infusion bottle. Normal saline is injected at a flow rate of 80-120 ml per minute to directly induce penile erection. The flow rate at the beginning of the erection is recorded, and then the flow rate is slowed down to maintain the erection. The speed of maintaining the erection is recorded. Finally, 100 ml of 30% diatrizoate is injected and films are taken under the monitoring of a fluorescent screen. After the filming, the needle is removed and local pressure is applied for 3-5 minutes. Conclusion: If you want to recover from cavernous body injury, you must first check the specific problem, so that you can better find the cause and treat it. We can try the two inspection methods introduced above. I hope the methods introduced above can help us treat cavernous body injury well. |
