Complicated urinary tract infection refers to recurrent or persistent urinary tract infection that causes severe damage to renal function when there are anatomical or functional abnormalities in the urinary tract system (such as obstruction, stones, etc.) or extrarenal comorbidities (such as diabetes, abuse of analgesics containing phenacetin, sickle cell disease, etc.).

Preventive measures

1. Urinate immediately after sex: Go to the bathroom immediately after sex. Even if the bacteria have entered the bladder, they can be expelled from the body through urination. 2. Urinate in time: When urinating, urine will flush away the bacteria in the urethra and vaginal opening, which has a natural cleansing effect.

3. Avoid contamination: The most common bacteria that cause infection is E. coli. Under normal circumstances, it parasitizes in the intestines and does not cause symptoms, but if it enters the urethra from the anus, it will cause urethritis. Therefore, after defecation, wipe with clean toilet paper, from front to back, to avoid contaminating the vaginal opening. If there are flushing facilities in the bathroom, it is best to flush the anus carefully.

4. Supplement vitamin C: Vitamin C can increase the acidity of urine, making it difficult for various bacteria that cause urinary tract infections to survive. Therefore, drinking more vitamin-rich drinks such as orange juice, citric acid, and kiwi juice is beneficial for preventing urinary tract infections.

5. Consult a doctor: Sometimes, even if you do everything you should do, you still get an infection. If you experience the above symptoms, consult a doctor as soon as possible. If the infection occurs frequently, such as 4-5 times a year, then do not ignore it. It is necessary to seek help from a doctor, develop a prevention or treatment plan, and work with the doctor to find out what causes the recurring infection.

It is recommended to use a three-day treatment, that is, oral sulfamethoxazole; or ofloxacin; or levofloxacin. Since the efficacy of single-dose therapy is not as good as the three-day therapy, it is no longer recommended. For areas where the resistance rate of pathogens to sulfamethoxazole is as high as 10% to 20%, nitrofurantoin can be used for treatment.

The treatment plan for complicated urinary tract infection depends on the severity of the disease. In addition to antimicrobial drug treatment, it is also necessary to deal with anatomical and functional abnormalities of the urinary system and treat other potential combined diseases. If necessary, nutritional support therapy is also required. If the condition is serious, hospitalization is usually required. First of all, underlying diseases such as diabetes and urinary tract infarction should be effectively controlled in a timely manner. If necessary, joint treatment with relevant professional doctors such as endocrinology is required. Otherwise, it is difficult to cure the disease with antibiotics alone. Secondly, based on experience, broad-spectrum antibiotics are used for intravenous treatment. During the medication period, the treatment plan should be adjusted in time according to changes in the condition and/or the results of bacterial drug sensitivity tests. Some patients still need combination medications, and the course of treatment is at least 10 to 14 days.
