(2) Blood cells: They include white blood cells and red blood cells. Under normal circumstances, there are no more than 10 white blood cells per high-power field of view, and they are scattered, not in piles or strings. Red blood cells are occasionally seen. In addition, epithelial cells, sperm, and starch granules can occasionally be seen in prostatic fluid. Prostatic fluid and semen are different, but they are closely related. Prostatic fluid is a component of semen and is mainly secreted by the prostate, while semen contains secretions from multiple glands. Semen is a mixture of sperm and seminal plasma. Sperm is a living cell produced in the testicular ducts, and there are many of them. Seminal plasma is composed of testicular fluid, epitesticular fluid, vas deferens peritoneal fluid, accessory gland secretion fluid and urethral gland fluid, including prostatic fluid. Prostatic fluid accounts for about 20% to 30% of seminal plasma, but the largest amount is the seminal vesicle secretion fluid, which accounts for about 60% to 70% of seminal plasma, and the rest of the ingredients account for only 10%. Seminal plasma is the necessary medium for transporting sperm, and it also contains substances necessary to maintain sperm life and can stimulate sperm motility. Semen contains many substances, such as high concentrations of organic substances, inorganic ions and various enzymes. Among them, many enzymes related to semen coagulation or liquefaction come from prostatic fluid, such as aminopeptidase, fibronectin, arginine ester hydrolase, etc. In addition, citric acid is all secreted by the prostate, and its function is to maintain the osmotic pressure of semen and the activity of sperm hyaluronidase. Your condition can be called prostatic discharge. Because a certain amount of prostatic fluid can be discharged with urine under normal circumstances, it is generally difficult to detect with the naked eye. In young people, due to the high level of androgen in the body, the amount of prostatic fluid secreted is relatively large. Frequent prostatic congestion can cause the gland duct to expand. During urination, due to the passive contraction of the prostatic smooth muscle, it is easy to cause prostatic fluid to overflow, especially at night, because the continuous passive erection of the penis further stimulates the secretion of glandular fluid, so that white urine appears when urinating in the morning. Or because the prostate is squeezed during defecation, intermittent white urine may appear during urination. In this case, if there are not many white blood cells in the prostatic fluid, if it is less than 10, it can be considered as prostatic discharge. However, in patients with chronic prostatitis, leukoplakia is also likely to occur due to inflammatory stimulation. At this time, more white blood cells will be found in the prostate, accompanied by certain clinical symptoms. The two can be distinguished. Another situation is excessive metabolism of oxalate and phosphate due to excessive exercise or excessive consumption of meat and vegetables, which results in salt crystal urine, which accumulates in the bladder and is discharged during urination, resulting in white urine. At this time, microscopic examination of the urine can reveal a large number of salt crystals, or a small amount of hydrochloric acid can be dropped into the urine, which will turn the turbid urine into clear, thus confirming the cause. |
