They are hemispherical cystic protrusions, skin color, smooth surface, soft and elastic to the touch, with thin walls like blisters, and the blisters are clear and translucent. Histopathological examination: The epidermis is normal, the cyst is located in the middle of the dermis, and it presents a round or irregular cystic cavity. The cystic cavity is empty or contains amorphous eosinophilic substances. The cystic wall is composed of one to several layers of short columnar or cubic epithelial cells. Some epithelial cells have transparent cytoplasm. In a few cystic walls, epithelial cells that secrete mucin can be seen, and melanocytes are occasionally seen. Connective tissue containing capillaries can be seen around the cystic wall, and the dermis is mildly edematous.

Diagnosis

1. Midline penile cysts are mostly located on the ventral side of the midline of the penis, especially at the glans penis.

2. The damage is mostly single, with a diameter ranging from a few millimeters to 2 cm.

3. It is usually a translucent cyst containing clear fluid.

4. Histopathological examination showed that the cyst wall was composed of one to several layers of short columnar or cubic epithelial cells, some of which had transparent cytoplasm. Connective tissue containing capillaries was visible around the cyst wall, and the dermis was slightly edematous.

Combining clinical manifestations with histopathological manifestations, diagnosis is not difficult.

Differential diagnosis

1. Penile apocrine cystadenoma

Apocrine sweat gland cystadenoma mainly occurs in middle-aged and elderly men, with the most common sites being the face, chest and shoulders, but occasionally occurring on the penis and foreskin. It is a translucent nodule with a smooth surface. Pathologically, it is not a retention cyst of the apocrine sweat gland, but a hyperplastic adenoma.

2. Dermoid cyst

Dermoid cysts mainly occur on the head, face and midline of the body, and rarely occur on the penis and perineum. They are firm, painless cysts that are hemispherical and protrude from the skin. More importantly, dermoid cysts have keratinocytes, mature hair follicles and sebaceous glands in their walls, which can be distinguished from midline cysts.
