How does [lumbar tuberculosis abscess] form?

An abscess appeared on the lumbar spine for no reason. Many patients are very puzzled as to how abscess can form when it is a bone disease. Therefore, in the early stage of abscess, many people did not realize that the appearance of abscess is inseparable from lumbar tuberculosis. So how does lumbar tuberculosis abscess form?

Cold abscesses formed after vertebral destruction can have two manifestations:

① Paravertebral abscess: pus gathers beside the vertebral body, which can be in the front, back or both sides. It is more common to accumulate in the sides and front. The pus lifts the periosteum and can also spread upward and downward along the ligament space, causing bone erosion on the edges of several vertebrae. It can also enter the spinal canal backwards and compress the spinal cord and nerve roots.

② Flowing abscess: When paravertebral abscess accumulates to a certain amount, the pressure increases and it will penetrate the periosteum and flow downward along the myofascial space, causing abscess in a location far away from the lesion. After the paravertebral abscess caused by lumbar spondylosis penetrates the periosteum, it accumulates in the psoas sheath to form a psoas abscess. Shallow psoas abscess can pass through the lumbar fascia to the lumbar triangle and become a lumbar triangle abscess. The lumbar triangle is a potential gap, and its edges are the posterior edge of the iliac crest, the outer edge of the sacrospinal muscle, and the posterior edge of the internal oblique muscle. Psoas abscess can also flow along the psoas major muscle to the lesser trochanter of the femur, becoming a deep abscess in the inguinal groove. It can also go around the back of the upper end of the femur, appear on the outer side of the thigh, and even flow down the fascia flexa to the area above the knee.
