What is condylomas?

Condyloma acuminatum is a reproductive system infection characterized by proliferative lesions in the anal genital area. Most cases occur in young people aged 18 to 50. The disease occurs after the pregnancy risk period, which lasts from a dozen days to eight months, with an average of three months. The disease is widespread and is mainly transmitted through sexual contact.

1. Genetic Testing Technology

So far, HPV cannot be detected by traditional virus culture and serological tests. The important laboratory diagnostic method is nucleic acid hybridization. The PCR method developed in recent years has the advantages of being specific, relatively sensitive, simple and rapid, and has developed an important path for HPV detection.

II. Histological examination of immune competence

The universal lactate dehydrogenase anti-lactate dehydrogenase method (PAP) shows the viral protein in the warts to confirm the presence of viral antigens in the wart lesions. When HPV protein is positive, a weak dark red positive phenomenon may appear in the superficial squamous epithelial cells of the warts.

3. Acetic acid white test

Apply 3-5% acetic acid to the warts for 2-5 minutes, the lesion area will turn white and slightly raised, and anal lesions may take 15 minutes. The basic principle of this test is the result of protein and acid coagulation and bleaching. The keratin produced by HPV-infected germ cells is different from that produced by normal uninfected squamous epithelial cells. Only the former can be faded by acetic acid. The acetic acid whitening test has a high sensitivity for detecting HPV, and it is better than basic tests in observing histological changes. However, sometimes in the case of epithelial keratinization or traumatic abrasions, positive and positive whitening signs appear to have irregular boundaries. The US CDC reminds that the acetic acid whitening test is not a specific test, and the positive result is relatively common.

4. Tissue analysis and chemical examination

Take a small amount of lesion tissue and make an acid-fast stain, using specific anti-human papillomavirus infection antigens for staining. If there are viral antigens in the lesions, the antigens and antibodies will bind. In the lactate dehydrogenase anti-lactate dehydrogenase (PAP) method, the nucleus can be stained red. This method has good specificity and is relatively rapid, which is helpful for diagnosis.

5. Pathological examination

The main features are incomplete keratin sclerosis, hypertrophic spinous layer, papilloma-like hyperplasia, sudden thickening and elevation of the epidermis, and the level of hyperplasia may be similar to that of occult squamous epithelial cell tumor. The thorn cells and the wall or material germ cells have a considerable number of nuclear divisions, which are quite similar to lesions. However, the germ cells are arranged in a regular manner, and the boundary between the hyperplastic squamous epithelial cells and the middle of the dermis is clear. Its characteristic is that the germ cells above the granular layer and the thorn layer have obvious vacuolation. This type of vacuolated germ cells are larger than normal, with light-colored cytoplasm, large and round in the center, and deeply alkaline nuclei. Generally, there is dermal edema, capillary dilation, and diffuse inflammatory infiltration with a high density around. Bushke-loewenstein is a very large or medium-sized wart. The epidermis grows very downward, replacing the underlying tissue and easily merging with squamous cells, so multiple puncture biopsies are required. If there is a tendency to develop slowly, it is a whole process of mild malignancy, which is simply called wart cancer.
