Communicating hydrocele, also known as congenital hydrocele, is caused by the processus vaginalis of the spermatic cord not closing after birth, which causes the fluid in the abdominal cavity to communicate with the fluid in the sac of the vagina, and the size of the hydrocele changes from time to time.

1. Injury: It is also a common cause of symptomatic hydrocele. It has been reported that 67% of patients develop hydrocele after renal transplantation, 35% after varicocele surgery, 0.9%-2.28% after hernia repair surgery, epididymisectomy, inguinal lymph node dissection, etc. can cause hydrocele due to stimulation of the spermatic cord and damage to the lymphatic vessels.

2. Infection: This is the most common cause. It is often caused by the invasion of tuberculosis, lb diplococcus and various non-specific bacteria such as E. coli, Staphylococcus, Streptococcus, etc., which can cause acute and chronic epididymitis, orchitis, spermatic cord inflammation, and hydrocele. Mumps virus infection can also cause it. Parasitic infections such as filariasis, which are prevalent in tropical areas, can also damage the lymphatic drainage of the tunica vaginalis, spermatic cord, and scrotum, leading to hydrocele.

3. Tumor: Pathological swellings in the testicle, epididymis, tunica vaginalis, spermatic cord, etc. can invade the tunica vaginalis, causing increased secretion and exudation or blocking the lymphatic system, resulting in hydrocele. The incidence of hydrocele in testicular tumors is 2% to 5%.

4. Certain systemic diseases: Water and sodium retention, circulatory stagnation, lymphatic obstruction caused by heart and kidney failure, liver disease, etc. can also cause hydrocele, often accompanied by systemic tissue edema or ascites. In addition, certain benign changes of the scrotum such as testicular torsion, testicular appendage torsion, spermatic cord varicocele, etc. can also induce hydrocele.

The symptoms of communicating hydrocele are the same as those of spermatic hydrocele, both of which are cystic masses in the spermatic cord without obvious symptoms. It is a congenital disease. The former is connected to the abdominal cavity through the processus vaginalis, and there is a small crack between the cyst and the processus vaginalis; the latter is not connected to the abdominal cavity, and the cyst is filled with exudative fluid, containing fibrinogen, etc., which can precipitate and block the above-mentioned crack or cause it to adhere, and even form multi-chamber septa in the cyst.

What are the symptoms of communicating hydrocele? The early symptoms of communicating hydrocele are cysts that can be large or small. As the course of the disease prolongs, the cyst often gradually increases in size and does not shrink. The later enlargement is mainly caused by the exudation of the cyst wall. If the cyst is infected, there may be local swelling and pain. The formation and anatomical relationship of the symptoms of communicating hydrocele and inguinal oblique hernia are the same. However, the diagnosis is different due to the different contents of the hernia sac. When the patient is standing, if the mass is seen protruding rapidly along the inguinal canal, sometimes an intestinal shape or intestinal tinea can be seen, it is a hernia. During the examination, the intestine can be touched. The spermatic cord is thick at the outer ring.
