Men's orgasm and ejaculation are not the same thing

Men can orgasm without ejaculating, and they can ejaculate without orgasming.

Although men can achieve erections and orgasms, there are reasons why they may not be able to ejaculate. These include surgical procedures that affect the reproductive system (such as prostatectomy for men with prostate cancer); obsessive compulsive disorder (such as being overly hygienic and believing that semen is extremely dirty); psychological/psychosexual issues (such as fear of partner pregnancy, fear of losing control, or cultural taboos); and adverse effects of some medications (such as antidepressants: 5-hydroxytryptamine selective inhibitors, SSRIs). Although SSRIs and other medications may interfere with sexual desire and function, depressed men should not stop taking them without consulting their doctor, as serious complications may occur. Some progressive neurological diseases such as multiple sclerosis, nerve damage caused by stroke or spinal cord injury, or hormone deficiency can also cause some ejaculation reflexes to occur.

On the contrary, sometimes men can also ejaculate without an erection or orgasm: including severe premature ejaculation (ejaculation without an erection or stimulation of the penis); psychological problems such as excessive excitement, tension, fear of intimacy, and sexual relationships; mental illness such as severe schizophrenia; complications of surgical operations involving the reproductive system; other conditions include hormone imbalances, progressive nervous system diseases, diabetes, drug or alcohol dependence, and brain or spinal cord damage.

Some men have the ability to separate ejaculation and orgasm at will. They are aware of the critical moment in their sexual response (the feeling that "ejaculation is inevitable") that they cannot control once they have passed it. They have successfully trained themselves to temporarily control their sexual response before the point. In this way, they delay the occurrence of ejaculation and are able to have more intimate time with their partner during sexual intercourse, thus obtaining the deep and intense pleasure that they cannot enjoy normally. An example of this practice is teaching tantric sexual techniques, where men learn to delay ejaculation as long as possible until they are fully prepared (have completed the tasks they should undertake).
