Is impotence treatable?

Many men do not have a proper understanding of erectile dysfunction and think that impotence cannot be cured, or they blindly follow some "miracle remedies" on the street and unreliable clinics, which makes the problem worse the more they try to treat it. So can impotence be cured? What methods can be used to treat it?

Whether impotence can be cured depends on certain criteria. For example, if normal sexual life is restored by certain external methods, such as inserting a penis prosthesis or taking PDE5 inhibitors orally, this can be understood as the cure of impotence. For example, if it can be restored to the past after treatment without relying on any external methods, it is also a kind of cure. So what kind of impotence can achieve this kind of ideal cure? It may be psychogenic impotence, which will be cured after treatment. It can also be complex impotence. Some of these impotence patients have psychological disorders, and some are caused by primary diseases such as diabetes, hypertension and other diseases that cause psychological problems and cause impotence. After treatment, it can also be cured. However, for impotence caused directly by primary symptoms, it is impossible to achieve ideal cure at present. Just like an antique car, it is impossible to restore it to the appearance of a new car after running for so many years. This means that it is not possible for such patients to "reverse" their impotence, at least at present, unless medical technology is more advanced in the future. Therefore, impotence is not completely incurable, but it depends on the type and group.

At present, the treatment of impotence can be divided into first-line treatment, second-line treatment and third-line treatment. Before the first-line treatment, there is also the treatment of primary symptoms. There is no contradiction between the treatment of primary symptoms and impotence under normal circumstances. For example, controlling the onset of the primary disease also improves the actual effect of impotence treatment.

First-line treatment for impotence: Oral drugs are the first-line treatment, mainly PDE-5 inhibitors, also known as Viagra, including tadalafil (Viagra), vardenafil (Ailida), and tadalafil (Cialis). The mechanism of action of this type of drug is all selective PDE-5 inhibitors, and the clinical efficacy is generally 70% to 80%. The incidence of side effects is generally 15%, with varying degrees, but there are very few serious unbearable reactions. Of course, there are also some drugs such as androgens.

One of the first-line treatments is negative pressure suction. It consists of a transparent plastic vacuum pump barrel, a mechanical pump, a connecting tube and a constriction ring. Put a vacuum pump barrel of appropriate size on the glans penis, shave the pubic hair to prevent it from affecting the closed-type effect, press the mechanical pump to produce negative pressure suction, and after the penis enlarges and gradually becomes hard, put the constriction ring on the penis to block the venous backflow and maintain the hard state. Finally, remove the vacuum pump barrel. Generally, the constriction ring is placed for a maximum of 30 minutes. Negative pressure suction equipment has the advantages of minimally invasiveness, few symptoms, unlimited application and wide acceptance. It is suitable for erectile dysfunction due to various reasons and can also be used for those who have failed surgical treatment, such as those who have had their prostheses removed. However, the actual effect is not very good for patients with penile cavernous body fiberization process and Perrot-type. It should be used with caution in patients with some coagulation disorders and those taking anticoagulants.

The second-line treatment for impotence is mainly some traumatic treatments such as penile corpus cavernosum injection. Penile corpus cavernosum injection is a relatively effective way to treat impotence. The injection method is not complicated. The patient can inject himself and have sex after the injection. The male genitals can start to expand 3 minutes after the injection and become hard in 5 to 8 minutes. However, this method is not without defects. After long-term injection, the fibrosis of the penile corpus cavernosum will be affected, and abnormal penile erection and other conditions may also occur.

The third-line treatment for impotence is mainly surgical treatment. The key is prosthesis insertion and vascular surgery. Surgical treatment can be performed on the dorsal side of the male genitals near the coronal sulcus, on the penis, on the vulva, at the junction of the male genitals and scrotum, etc. In surgical treatment, the tunica albuginea of the corpus cavernosum of the penis is cut open, and a probe is used to dilate and accurately measure the size of the penis to select a suitable prosthesis. After the prosthesis is inserted, the tunica albuginea is sutured. When the inflatable prosthesis is inserted, a centrifugal water pump is placed in the scrotum and a water storage bag is placed in the anterior fossa of the bladder. The third-line treatment is suitable for some patients who have failed to inject the corpus cavernosum of the penis or are intolerant to PDE-5 inhibitors. For some patients with heart disease, taking PDE-5 inhibitors is still harmful.
