Once BPH patients are diagnosed and assessed, doctors should inform them that there are several different treatment options available. Patients should also consult with their doctors and seek their specific guidance to choose a treatment that is easy to use and has fewer side effects.

Self-treatment should be strongly recommended to patients. Patients with mild symptoms (symptom score 0-7 points) only need to wait and observe. McConnell et al. emphasized that patients with recurrent urinary retention (at least 1 recurrence of urinary retention after catheter removal), recurrent UTI, gross hematuria, combined bladder stones, renal insufficiency and extremely large bladder diverticulum are absolute indications for surgery.

1. Wait and see. There are not many scientific studies on the presenting history of BPH. The progress of the presenting history and the risk factors for the onset of symptoms are still uncertain. There is no doubt that for patients with BPH who have symptoms, the development trend of the disease is sometimes difficult to avoid, but the symptoms of some patients may be relieved or disappear automatically.

Retrospective studies of the current medical history of BPH are prone to errors, which are related to the selection of medical records and the type and level of follow-up, and there are very few reports of retrospective analyses. Selecting to wait and see does not mean to tolerate the development trend of the disease and wait passively. A comprehensive assessment should be conducted once a year, including IPSS, DRE, routine urine examination, renal function measurement, urodynamic model examination, B-ultrasound and necessary imaging diagnostic examinations. When in doubt, measure the blood cell PSA level to alert the occurrence of prostate cancer (Pca). A series of patients were observed for 2 to 2.5 years without treatment, and 33% to 60% of the symptoms improved. Many patients were in a stable condition, and 30% to 45% of the patients required surgery. In another group of 112 BPH patients, about 38% needed prostatectomy, while in 945 patients without LUTS, only 8% needed surgery. The most predictive analysis of surgical treatment results is the change in male prostate volume and Qmax. The risk of surgical treatment by age is about 10% at 50 years old and about 50% at 70 years old.

As mentioned above, observation and waiting is suitable for patients with mild symptoms (score 0-7). Patients with mild to moderate symptoms can also try it if they persist, but it is best to follow up the diagnosis period and the opportunity for treatment intervention.

Prostatic hyperplasia (BPH), also known as prostatic hyperplasia in ancient times, is one of the common diseases in elderly men and is a kind of benign prostatic disease in men. The cause of the disease is related to the imbalance of male hormones and female hormones in the human body. The disease begins in the glandular tissue, connective tissue and smooth muscle tissue of the middle or lateral lobe under the mucosa of the posterior urethra, producing a mixed spherical mass.

The hyperplasia of the lateral lobes and mid-section is prominent, invading the bladder or urethral opening, compressing the bladder neck or urethral opening, causing lower urinary tract obstruction. When prostatic hyperplasia causes obstruction, the detrusor muscle of the bladder becomes thicker, and the mucosa has posterior capsules, chambers and diverticula. Long-term urinary obstruction increases the bladder height-to-width ratio, softens the bladder wall, increases bladder air pressure, and loses its valve function at the tail end of the urethra, causing bladder urethral reflux.
