The skin lesions are one or several round or oval patches of coin-sized size, lighter in color than the surrounding normal skin, pale white, dry on the surface, with a small amount of fine gray-white scales, and mild or no inflammation on the base. The lesions may gradually expand, and adjacent ones may merge with each other. There may be slight itching or no symptoms.

The lesions usually occur on the face, especially the cheeks and forehead, but can also be seen on the neck, trunk, and limbs. The lesions may resolve spontaneously, leaving mild hypopigmentation. This disease is more common in children or adolescents, but can also occur in adults. It occurs more frequently in spring, but can also be seen in early summer and winter.

Typical white spots are the size of a fingernail to a coin, nearly round or elliptical. Although they may expand or merge into large irregular patches later, the edges of the white spots are always visible no matter how their shape changes, and there are island-like pigment spots in the white spots. Another typical type of white spot is the band-like or cord-like depigmented spots distributed along the nerves, and the edges of the white spots are as neat as if cut by a knife.
