When the penis is erect, the vascular sinus is congested, the internal pressure increases, and the tunica albuginea thins to only 0.2-0.25 mm, making it very easy to rupture. The rupture is usually horizontal, generally not exceeding 1/2 of the circumference of the corpus cavernosum, and most of them are located at the far 1/3. This kind of injury is relatively rare, because it has the hidden danger of being difficult to explain.

Causes

Penile fracture often occurs when the penis is erect, such as when sexual intercourse goes wrong and the penis hits the woman's pubic symphysis or perineum, or when masturbation breaks the penis and causes injury. According to domestic statistics, these two reasons account for 45% and 38% respectively. About 10% of cases are caused by the erect penis being squeezed on the bed, and the other 7% are due to other reasons.

When the penis is in an erect state, the tunica albuginea and corpus cavernosum are ruptured by direct external force. The tunica albuginea is one of the toughest fascia in the body, but when the penis is in an erect state, the blood flow to the corpus cavernosum increases by 7 to 8 times, causing the tunica albuginea to stretch and become thinner (1/8 to 1/4 of the thickness when not erect) and lose its elasticity. At this time, it is very easy to be ruptured by vertical external force or even slight external force.

Clinical manifestations

Patients usually complain that during sexual intercourse, the penis is twisted, with a sound when it is broken, accompanied by severe pain, and then the penis erection disappears. The penis becomes soft, swollen, bent and deformed and leans toward the healthy side. Due to the rupture and bleeding of the corpus cavernosum and tunica albuginea, blood edema and ecchymosis are formed, and the penis skin becomes bluish purple. Urinating is generally normal.

For a small number of patients with mild symptoms, cavernography, ultrasound, and magnetic resonance imaging (MRI) are required for a clear diagnosis. During the diagnosis, it is necessary to distinguish whether the corpus cavernosum ruptures on one side or both sides, whether there is urethral damage, whether it is simple skin damage, vascular damage, or penile transposition.

Complications

The complications of conservative treatment of penile fracture are as high as 30% to 44%, including missed diagnosis of urethral injury, persistent hematoma, secondary infection, penile fibrosis and angulation deformity, painful nodules, and resulting sexual dysfunction. Patients who choose early surgical treatment do not have the above complications.

1. Non-surgical treatment

Treatment measures include sedation and analgesia, indwelling urinary catheter, penile pressure bandage, local cold compress first, hot compress after 24 hours, oral estrogen, intravenous antibiotic treatment, non-surgical treatment of penile swelling that subsides slowly, and local scar nodules, as well as complications such as penile erectile dysfunction and painful erection.

2.Surgery

Rupture of the corpus cavernosum of the penis should generally be treated as soon as possible to avoid complications. Treatment measures include: removing blood swelling, suturing the ruptured tunica albuginea with silk sutures, and stopping bleeding surgically. Generally, the penis swelling subsides within 10 days after the operation, and sexual function recovers well after the operation.
