Many things in life have to go through the first time, and the same is true for sexual life. Some people get hard and soft the first time. There are many reasons for this phenomenon, the main ones are mental and psychological factors. Because the first time is mentally tense, it is easy to cause premature ejaculation, so it will get hard and soft again quickly. Sometimes it is also directly related to environmental factors. If the environment is not very comfortable, it will increase mental tension.

1. Treat erectile dysfunction medically. If you have tried changing your lifestyle and are still struggling to maintain an erection, make an appointment with your healthcare provider. ED can be caused by type 2 diabetes, high blood pressure, certain prescription medications, stress, or even being overweight.

Your doctor will check your circulation, examine your penis and rectum, test your nervous system, and ask you questions about your medical history, such as how long you've had trouble maintaining an erection.

Your doctor will assess your cardiovascular risk factors as mild, moderate, or severe and make sure your heart is healthy enough for sex.

Based on your personal health history, your doctor can help recommend the right treatment plan to address ED. For example, if you have low testosterone levels, your doctor may recommend a testosterone supplement.

If you're relatively healthy, your ED may be classified as psychogenic, meaning there's a mental or emotional disorder preventing you from getting and/or maintaining an erection.

2. Get a blood test to assess your testosterone levels. Testosterone naturally peaks during puberty and adolescence and declines with age. If a blood test shows that you have low testosterone levels, this is likely the culprit behind your erectile dysfunction. Your doctor may first recommend natural lifestyle changes, such as losing weight or gaining muscle mass. If your testosterone levels are lower than average for your age, they may prescribe testosterone supplements. [12]

If your testosterone levels are low due to aging, remember that this is completely normal. There is currently no recommendation to take testosterone as a way to solve this problem.

3. Consider oral medications. Your doctor may prescribe medications that increase blood flow to your penis, thereby helping you maintain an erection. Medications commonly used to treat ED include Viagra, Cialis, and Levitra.[13]

If your doctor prescribes Cialis, your doctor will likely recommend that you take 10 to 20 mg at least 30 minutes before sexual activity. You should not take the medication if you have experienced severe overactivity with your medication or if you are using nitrates (such as nitroglycerin) to treat chest pain.

If your doctor prescribes Levitra, you need to take it 60 minutes before sex with or without food. This medication should not be taken with nitrates.

4. Investigate mechanical aids. You can also ask your doctor about using mechanical aids to help you achieve and maintain an erection. Some men use vacuum devices and constriction rings to help with erections. The vacuum is placed on the penis and draws air, which draws blood into the penis and causes an erection.

This is maintained by placing a band or ring at the base of the penis, which keeps it erect for 30 minutes.

However, this can be an uncomfortable and awkward way to treat ED.

5. Use penile injection therapy. Another option you might suggest to your doctor is penile injection therapy. For this, you will be trained by your doctor on how to inject medication into your penis to relax the blood vessels and increase blood flow leading to an erection. This treatment has been shown to be effective in treating a range of physical and psychological problems.

Potential side effects include scarring and the risk of persistent and painful erections if not administered properly.

Injection therapy may cause high blood pressure and dizziness.
