Abdominal distension due to vas deferens

The vas deferens is an important male tissue. When vas deferens inflammation occurs, semen is often unable to pass through the vas deferens, which is an important cause of male infertility. Generally speaking, simple vas deferens inflammation is relatively rare, and other inflammatory symptoms often appear at the same time, such as prostatitis, seminal vesiculitis, orchitis, etc. In more serious cases, it can also cause abdominal distension. Clinical manifestations

(1) Acute vas deferens causes swelling and pain in the scrotum on the affected side, red and swollen skin, and pain radiating to the abdomen and thigh root on the same side, often causing the patient to bend over and hold his abdomen. There is local tenderness in the scrotum and obvious tenderness in the vas deferens. In severe cases, there may be fever and purulent lesions may form around the vas deferens.

(2) Chronic vas deferens causes swelling, pain or redness on the affected side of the scrotum, with pain radiating to the abdomen and thigh. The clinical symptoms are milder than those of acute vas deferens, with a slow onset and a history of recurrent attacks. It is often accompanied by testicular and epididymitis. Physical examination shows that the scrotal section of the vas deferens is thickened and hardened. In severe cases, the vas deferens is adhered to the surrounding area, the levator testis is tense, and the scrotum and testicles are retracted. Vas deferens nodules that occur after vas deferens injury or vasectomy develop from the nodule to both ends, with the vas deferens thickened or adhered. The nodules can be painful or asymptomatic.

Most cases of vas deferensitis are caused by bacterial infection. After infection, bacteria rapidly multiply in the vas deferens and spread along the vas deferens to the distal testis, causing seminal vesiculitis, prostatitis, etc. Spreading to the proximal testis causes acute epididymitis. The infection route is mainly through blood, lymph invasion and surgical incision.

1. Hematogenous infection: The vas deferens artery originates from the internal iliac artery and divides into ascending and descending branches at the base of the bladder. The ascending branch follows the vas deferens through the inguinal duct to the testis and anastomoses with the testicular artery. The descending branch goes to the vas deferens pot and seminal vesicle. Bacteria can reach various sections of the vas deferens along the artery.

2. Lymphatic infection: The upper part of the vas deferens lymphatic vessels is connected with the spermatic cord lymphatic vessels, and the lower part is anastomosed with the seminal vesicle lymphatic vessels. Inflammation of adjacent organs often invades the vas deferens through the lymphatic vessels and causes vas deferensitis.

3. Surgical incision infection: due to lax disinfection before and after male sterilization surgery, or bacterial infection induced by surgery, it directly causes vas deferens and thus affects the related reproductive organs.
