Chronic scrotal eczema: There is often an acute blisters phase. Even if it is chronic at the beginning, blisters are often seen in tissue pathology. In terms of the cause of the disease, the former is mainly allergic, and the latter is mainly mental disorder. 2. Pruritus: At the beginning of the disease, it is just itching, without any primary rash, accompanied by burning, crawling, and ant-walking sensations. The affected area often has secondary damage such as scratches, rubbing, exudation, and blood scabs. The boundaries of lichenification are unclear. Scrotal dermatitis is caused by local itching and scratching, and polygonal flat papules merge into pieces. The boundary between lichenification and normal skin is clear. 3. Lichen planus: polygonal rash or triangular flat papules with a depression in the center, purple or dark red, waxy luster and strip-like damage. The cheek mucosa often has grayish white flat polygonal rash. Scrotal dermatitis does not have these manifestations. Categories There are three types of scrotitis 1. Erythematous type: Symmetrically distributed on both sides of the scrotum, about 3×4 cm in size, with distinct erythema edges, covered with gray or brown thin scabs, and a rough surface. After the scabs are peeled off, slightly reddish skin is exposed, without significant infiltration. When the lesions expand, they may fuse at the root of the penis. 2. Papular type: At the beginning, there are several scattered soybean-sized papules on one side of the scrotum, which are significantly higher than the skin, flat, and covered with thin brown scabs. Later, they increase and expand, densely and symmetrically distributed on both sides of the scrotal suture, about 3×4cm, fused into a piece, and often fused at the root of the scrotum. There are irregular, gray-black thick scabs that are significantly higher than the skin surface in the coronal sulcus of the penis, and there are gray scales on the penis. There are scattered folliculitis or abscesses in the pubic area and groin area. 3. White papules with silver scales: This type is rare but very typical. It consists of a large area of flat papules the size of melon seeds fused together in front of the scrotum. They are silvery white in color and silvery scales will fall off when scratched. Treatment and prevention treatment 1. Drug treatment (1) Compound levofloxacin solution is a preparation made in-house in hospitals. It is mildly irritating and has a rapid effect. It can be applied externally 1 to 2 times a day for 10 consecutive days to achieve clinical recovery. (3) 1% clotrimazole, clematis cream, 1% econazole cream, etc. I recommend clotrimazole.
