Generally, testicular cysts do not require treatment, but surgical removal can be used when necessary. Smaller cysts do not require treatment and can be observed regularly. After puncture and aspiration, anhydrous ethanol or other hardening agents are injected. Surgical removal of cysts is suitable for those with larger cysts and obvious symptoms. The common site of epididymal cyst is the epididymal head, while it rarely occurs in the body and tail. It originates from the epithelial cells of the efferent duct of the rete testis. The cyst fluid often contains sperm. The clinical manifestations are mostly a feeling of scrotal swelling, no special discomfort, and the lesion develops slowly. A round or oval mass can be felt in the epididymal head. It has a smooth surface, no tenderness, and a cystic sensation. It has clear boundaries with the surrounding tissues and no adhesions.

If the cyst is small and asymptomatic, it can be observed first, and no treatment is needed, and it generally does not affect fertility. If the symptoms are severe or the cyst is large and the patient is married with children, surgical removal can be performed. Surgery may damage the epididymis and affect fertility, so unmarried patients should be cautious.
