What is scrotal pain?

Diagnosis of scrotal pain

(I) Traumatic scrotal pain: There is a history of scrotal trauma, which presents as persistent pain or swelling, and gradually worsens. Examination shows that the scrotal skin is congested and swollen, the scrotum is enlarged and cyanotic, and there may be tenderness.

(ii) Infectious scrotal pain often occurs suddenly, with persistent swelling or throbbing pain. If the scrotum itself is infected, the scrotal wall may become red and swollen, and in severe cases, there may be erosion or skin peeling. If the contents are infected, the corresponding parts may become swollen and tender to touch.

(III) Tumor-related scrotal pain Tumors generally develop slowly and only cause local swelling and pain when they grow to a considerable size. Examination may reveal masses in the corresponding organs. Testicular hydrocele, varicocele, inguinal hernia, etc. can also cause scrotal pain.

In the early stage, it may be caused by local hematoma or infection, and later it may be caused by painful nodules or epididymal congestion. It may occur gradually after vasectomy, mostly bloating or pulling pain, sometimes radiating along the vas deferens to the groin or lower abdomen, and worsening after sexual intercourse.

(V) Scrotal necrosis is characterized by: ① Clinical manifestations: history of scrotal injury, infection and systemic disease. Severe scrotal pain, accompanied by chills, high fever, nausea, vomiting, fatigue, and even mental symptoms such as delirium. Examination shows that the scrotal skin is red, hot, hard, and swollen, with a creaking sound or a twisting sensation, necrosis of the scrotal skin or tunica vaginalis, bleeding from the wound, and in severe cases, the contents of the scrotum are exposed. ② Auxiliary examination: Routine blood tests show an increase in the total white blood cell count and neutrophil count, and the wound bacteria can culture hemolytic streptococci, Pseudomonas aeruginosa (also known as green bacillus), Staphylococcus aureus, and aerobic streptococci. Pathological examination showed acute cellulitis.
