1. Diagnosis

There are many laboratory tests for mucosal diseases, which should be considered in combination with the cause. For example, infectious diseases can be diagnosed with bacteriological tests, viral diseases can be identified by viral culture, and tissue pathology can provide a basis for the diagnosis of tumors. Sexually transmitted diseases also have many tests, such as syphilis serological tests, PCR tests can be used to diagnose a variety of diseases, and mycological tests are helpful for the diagnosis of fungal diseases.

2. Etiology Identification

(1) Genital ulcer diseases

1. Behcet's disease

This disease is also called eye, mouth, and genital syndrome. The cause is unknown, and the basic pathological change is small vessel inflammation. The mucosal ulcers of this disease are often multiple, painful, and recurrent. The ulcers are generally small, ranging from millet to soybean size, and may have yellow-white secretions. Systemic symptoms may include low fever, joint pain, etc., and may affect the digestive tract, heart, brain tissue, etc.

2. Acute vulvar ulcer

It is an acute painful ulcer that occurs in the vulva. It is more common in unmarried women. There are three types of ulcers: gangrenous type, venereal type, and miliary type. Gangrenous type often has systemic symptoms such as high fever, while venereal type and miliary type usually have no systemic symptoms. The subjective symptoms of ulcers are burning and pain, which may be accompanied by nodular erythema of the lower limbs and oral ulcers. Therefore, some people believe that it is the vulvar manifestation of auto-obstruction disease. The course of the disease is generally several weeks, and it often relapses.

This disease has no history of unclean sexual intercourse, ulcers that are painful, tender, and recurrent, which can be distinguished from sexually transmitted diseases such as chancroid and soft chancroid.

3. Genital herpes

Male herpes occurs on the penis, glans penis, coronal sulcus and other parts, rarely involving the scrotum. Female herpes is common on the labia, clitoris, perianal area, vagina and even cervix. There is often a burning sensation at first, and then clusters of small water scars will occur. After a few days, they will rupture and erode, forming shallow ulcers with a soft base. The course of the disease is short and easy to relapse. Pregnant women with genital herpes are prone to miscarriage, premature birth, and can also infect newborns.

This disease can be distinguished from Behcet's disease, chancre, and acute vulvar ulcers by the formation of ulcers after the rash, soft texture, and short course of illness.

4. Chancroid

This disease is one of the sexually transmitted diseases caused by Haemophilus ducreyi. It is characterized by multiple painful soft ulcers in the genitals, which are round or oval in shape, with irregular or saw-like edges, uneven base, light yellow purulent secretions on the surface, and easy bleeding.

5. Chancroid

The characteristic is a single or a few painless hard ulcers in the genitals, which do not fester, have clear and neat borders, and nearby lymph nodes are enlarged. They heal in 2-3 weeks. They usually occur on the glans penis, coronal sulcus, and penis in men, and on the labia majora and minora, and vagina in women. The lips, tongue, anus, and fingers are also common sites.

6. Lymphogranuloma venereum

It is caused by Chlamydia trachomatis infection, a sexually transmitted disease. The destructive characteristics are painless, shallow, soft, round ulcers with clear borders. There may be a small amount of secretions on the surface. It is usually solitary and mostly occurs on the glans penis and the vulva of men and women.

7. Inguinal bud swelling

This disease is Donovan bacteria infection, a sexually transmitted disease characterized by painless soft ulcers in the vulva, groin, anus, etc., which proliferate, bleed easily, are beef red in color, and have foul-smelling, slurry-like purulent secretions. Multiple satellite-like lesions may appear around the ulcers.

(2) Other diseases of the external genitalia

1. Pearly papules of the penis

The cause is related to the stimulation of secretions from the foreskin, which is damaged into small papules that occur on the edge of the male coronal sulcus near the glans. The round papules are the size of a needle head and surround the glans, with no spontaneous symptoms.

2. Flat warts

Flat warts are secondary syphilis rashes and a sexually transmitted disease. They are commonly seen around the anus, vulva, armpits, and groin. They are large, flat-topped papules that are pale white or light red in color. They can fuse together to form flat, proliferative protrusions that are soft and lobed. The surface is stained and Treponema pallidum can be found on the surface of the secretions.
