Causes of Epididymitis What are the causes of epididymitis? Pathogenic bacteria retrogradely enter the epididymis, urine reflux, surgical infection, and mumps can all cause epididymitis. 1. Pathogenic bacteria retrogradely enter the epididymis. The most common pathogenic bacteria of epididymitis are Escherichia coli, and Chlamydia trachomatis can also cause acute epididymitis. Pathogenic bacteria often retrogradely enter the epididymis through the vas deferens. 2. Urine reflux. Urine can reflux into the seminal vesicles. Sterile urine reflux can cause urine to enter the ejaculatory duct, leading to chemical epididymitis, and can also be secondary to prostatitis or urinary tract infection. 3. Surgical infection. When the inflammation affects a larger area, both the epididymis and testicles will swell, and the boundary between the two will be unclear, which is called epididymo-orchitis. The spermatic cord on the affected side will be thickened and tender. Under normal circumstances, the acute symptoms will gradually subside after a week. 2. Symptoms of chronic epididymitis (1) Scrotal pain. Scrotal pain is a typical manifestation of chronic epididymitis, often manifested as long-term or intermittent pain in the affected side of the scrotum. The pain is usually dull and there is a feeling of distension. (2) Lower abdominal pain. The pain caused by chronic epididymitis can radiate to the lower abdomen and the ipsilateral groin. Patients may experience pain in the groin or lower abdomen. (3) Thickening of the vas deferens. This is usually manifested as thickening of the vas deferens on the same side. (4) Hydrocele.
