The testicle is an important part of the male reproductive system, and testicular rupture is a relatively common condition. This may be caused by external damage to the testicle. The testicle is hit or impacted by the outside world, which may easily affect normal fertility. It can be treated through traditional Chinese medicine conditioning and Chinese medicine treatment.

What to do if the testicle ruptures?

1. There is severe pain at the time of injury, but it returns to normal the next day. This is because the nerve endings on the surface of the testicle are more abundant and sensitive, and there is little internal damage, which does not affect fertility.

2. The testicles are severely injured, with mild local congestion that subsides after a few days. The injury is local damage or subcutaneous blood vessel damage, and fertility will not be affected after repair.

3. Severe testicular rupture. There are blood spots at the injured area, blood swelling in the scrotum, and obvious pain and tenderness. After surgical repair, some fertility functions are retained. Although the testicles of a few people can heal naturally, they completely lose their fertility functions due to fibrosis.

4. Some people's testicular damage is not serious, but the blood-testicular barrier is damaged, allowing sperm antigens to enter the blood circulation and produce anti-sperm antibodies, which causes sperm to agglutinate or stop and lose the ability to fertilize, affecting fertility.

1. Syndrome differentiation and treatment:

This disease is mainly divided into two stages: early and late.

(I) Bloody wound (early stage)

Treatment method: stop bleeding, dissipate blood stasis, relieve swelling and relieve pain. The prescription often uses Shihuisan combined with Huaruishisan with modifications. In the prescription, the main hemostatic ingredients are Artichokes, Platycladus orientalis, Rubia cordifolia, and Xueyutan. The charcoal is fried to enhance its hemostatic effect; Rhubarb, Cortex Moutan, Rhizoma Cibotii, and Pollen Pu can promote blood circulation and dissipate blood stasis, clear away heat and cool blood; Huaruishi and Panax notoginseng are holy medicines for stopping bleeding and dissipating blood stasis; Corydalis, Toosendan, Frankincense, and Myrrh are added to regulate qi and dissipate blood stasis, relieve swelling and relieve pain. The combination of these medicines can achieve the effect of stopping bleeding, dissipating blood stasis, and relieving swelling and pain. If there is a tendency of heat generation, add dandelion, honeysuckle, phellodendron, and raw rehmannia to clear away heat and cool the blood; if the bleeding has stopped, remove the artichokes, cypress leaves, charcoal, and palm bark, and add angelica, red peony root, ligusticum chuanxiong, and safflower to increase the effect of promoting blood circulation and removing blood stasis.

(II) Blood stasis syndrome (late stage)

Treatment: Promote blood circulation and dissipate blood stasis, unblock meridians and disperse nodules. For details on the treatment of testicular atrophy, see this section. Prescription: Fuyuan Huoxue Decoction combined with Taohong Siwu Decoction with modifications. In the prescription, Chinese angelica, salvia miltiorrhiza, red peony root, peach kernel, safflower, raw rehmannia root, Chuanxiong and other herbs promote blood circulation and dissipate blood stasis; rhubarb breaks up blood stasis; pangolin, zelan and Vaccariae liuli promote blood circulation and disperse nodules; bupleurum and chuanxiong lead the herbs into the liver and go down directly to the affected area; leech is added to break up blood and remove blood stasis; oyster is added to soften and disperse nodules. All the herbs are used together to promote blood circulation and dissipate blood stasis, unblock meridians and disperse nodules. If the deficiency of qi is obvious, astragalus, dangshen and other qi-invigorating herbs can be added; if the scrotum feels cold, fennel, cinnamon bark, kudzu vine and other herbs that warm the meridians and dissipate cold can be added.

(III) Single prescription treatment

1. Take 1g of amber powder twice a day, mixed with honey.

2. Take 1.5g of Panax notoginseng medicine or Zhishang powder twice a day, mixed with rice wine.

3. Huoxue Sanyu Decoction: 6g each of Angelica root, red peony root, peach kernel, and wine-fried rhubarb, 5g each of Chuanxiong and Sumu, 3g each of Moutan bark and fermented aurantium with wheat flour, and 2g of Betel nut. Decoction in water, 1 dose per day. Suitable for late stage of testicular injury, hematoma and swelling that does not go away.

(IV) External medication

1. Apply an appropriate amount of Zhishangsan or Shensanqi powder to the affected area with cold boiled water, and change the dressing twice a day. It is suitable for the early and middle stages of testicular trauma.

2. Apply an appropriate amount of Yunnan Baiyao to the wound or apply it to the affected area with cold boiled water, 1 to 2 times a day. Suitable for the early stage of testicular injury.

3. Take 10g each of Luodeda, safflower, raw pinellia, bone-cutting herb, 6g of licorice, 15g of scallion root, boil 1000ml of water, add 100ml of vinegar, boil again, fumigate and wash the affected area, 2-3 times a day, 10-15 minutes each time. It is suitable for the stage of testicular hematoma organization.
