What are the TCM diagnosis and treatment methods for prostatitis?

In daily life, prostatitis is a common disease of the male reproductive organs. With the continuous development of living standards in real society, the incidence of prostatitis is also gradually increasing. Suffering from prostatitis can easily cause great harm to physical health and easily lead to sexual function problems. It is relatively safe to diagnose prostatitis with traditional Chinese medicine, and traditional Chinese medicine treatment methods can also be used.

1. Wet and hot injection type:

Symptoms: Painful and streaky urination, abdominal cramps, bloating and pain in the perineum, turbid white fluid dripping from the urethra, yellow and greasy tongue coating, and slippery and rapid pulse.

Causes: Damp heat is one of the most common causes of prostatitis. Damp heat stops bleeding and can invade from the outside or from the inside. Exogenous damp heat and fire poison, which are not dispersed, cause damp heat and turbid evil to flow downward; or the vulva is unclean, the foreskin is too long, which harbors dirt, or the sexual intercourse is unclean, the damp heat evil invades from the lower part of the body, stays in the sperm chamber, mixes the clear and turbid, and the sperm leaves its place, causing this disease. Endogenous ones can be caused by eating fatty, sweet, wine, cheese and spicy foods, which accumulate dampness and generate heat, and flow downward to the bladder, leading to the occurrence of this disease.

Treatment: Clear away heat and promote diuresis. The prescription is Ba Zheng San with modifications: Akebia, Plantago, Polygonum multiflorum, Dianthus superbus, Talcum, Fructus cyrtonemae, Rhubarb, and Licorice.

2. Qi stagnation and blood stasis type:

Symptoms: stagnant urine, distension and pain in the perineum and lower abdomen, enlarged and hard prostate, dark purple tongue, and stringy and astringent pulse.

Causes: Damp-heat pathogens cannot be cleared for a long time, the Yang Fire is suppressed for a long time, the sperm path is blocked and the sperm is stagnant; or due to emotional imbalance, depression damages the liver, and the liver fails to release; the circulation of Qi and blood is not smooth, the meridians are blocked, and Qi and blood are stagnant; or due to being affected by cold pathogens, the blood is entangled, the Jueyin meridian is damaged, Qi is stagnant, blood is stagnant, and the circulation is not smooth, thus the disease occurs.

Conditioning: It is advisable to promote blood circulation and remove blood stasis, promote qi circulation and unblock the meridians. The prescription is Shaofu Zhuyu Decoction with modifications: peach kernel, safflower, angelica, fennel, Toosendan fruit, ebony root, red peony root, orchid, and dandelion.

Symptoms: There is often white turbidity at the urethral opening, swollen perineum, soreness of waist and knees, hot flashes and sweating, red tongue with little coating, and weak pulse.

Causes: Excessive sexual intercourse, or holding back semen, drinking, sex, and fatigue, which damage the essence and qi, resulting in damage to the kidney essence and agitation of the fire. Long-term illness that damages the kidney or yin deficiency in the body, imbalance of water and fire, yin deficiency and excessive fire, which disturb the sperm chamber, can also lead to the occurrence of this disease.

Conditioning: It is suitable to nourish the liver and kidneys and clear the fire. The prescription is selected from Zhibai Dihuang Decoction with additions and subtractions: Anemarrhena, Phellodendron, Radix Rehmanniae, Radix Angelicae Dahuricae, Cortex Moutan, Poria, Radix Aconiti Lateralis Preparata, Polygonatum sibiricum, Radix Bletillae Sinensis, and Radix Salviae Miltiorrhizae.

4. Kidney Yang Deficiency Type:

Symptoms: Stretchable and thick urine with semen, fear of cold, soreness and coldness in waist and knees, impotence, premature ejaculation, pale and fat tongue, and deep and weak pulse.

Causes: In the early stage, it is mostly due to kidney yin deficiency, excessive movement of yang fire, followed by damage to yin and yang, or congenital yang deficiency, insufficient kidney qi, loose sperm gate, sperm leaving its place, yin essence becoming corrupt and turbid, waste sperm flowing out, and gradually causing turbid sperm.

Conditioning: Warming the kidney and strengthening yang, the prescription is Jingui Shenqi Pills with modifications: processed aconite root, dodder seed, phenanthera, eucommia bark, polygonatum, angelica root, yam, and poria.

Symptoms: cloudy urine, sallow complexion, drowsiness, loss of appetite, pale tongue with white coating, and weak pulse.

Cause: Spleen and kidney deficiency, or long-term illness that damages the spleen and kidney. The spleen is responsible for ascending the clear, and the kidney is responsible for sealing and storing. If the spleen is deficient, dampness will be more difficult to transform, and if the kidney is deficient, essence will easily leak downward, resulting in dysfunction of ascending the clear and descending the turbid, and the clear and turbid cannot be distinguished, and the damp and turbid flow downward, causing this disease.

Conditioning: It is advisable to strengthen the spleen and benefit the dampness. The prescription is Shenling Baizhu Powder with modifications: Dangshen, stir-fried Baizhu, Poria, Coix, Amomum, Zexie, Angelica, Kuncao, Chenpi.
