A normal penis is straight after erection, with a slight bend that is not very obvious. This is a normal phenomenon. The penis itself is soft and can bend freely. If the penis bends under abnormal conditions, it is necessary to consider an abnormality of the tunica albuginea of the corpus cavernosum. In current clinical practice, it is necessary to identify the cause and choose an appropriate treatment method. Surgery is the most common. Early surgery is recommended as long as it is during the adolescent development process to avoid developmental abnormalities. 1. What is penile curvature? The medical name for penile curvature is "abnormality of the corpus cavernosum albuginea", which means that there is more tunica albuginea in some parts, thus pulling the penis to the other side. For example, if there is more tunica albuginea on the right side, the penis will bend to the left. 2. Causes of Penile Curvature: A normal male penis consists of two parts, the corpus cavernosum, the urethra and the corpus cavernosum. When sexual desire is high, the corpus cavernosum is filled with blood and the penis becomes erect. If a person has penile curvature, the penis often cannot erect normally when sexual desire comes, thus affecting sexual function. At the same time, the urethra opening of such patients moves backwards, and the clothes and pants are often wet when urinating, which brings great inconvenience to life and must be treated. Clinically, the most common diseases that cause penile curvature in men are hypospadias, penile sclerosis (or chronic cavernitis), and simple penile curvature. 3. The best time to perform corrective surgery: Hypospadias is a congenital malformation, which can sometimes occur for several generations in a family. In mild hypospadias, the urethra opens at the coronal sulcus of the penis, and the penis itself is not curved or slightly curved. It is often ignored by parents in childhood, and it does not affect sexual function when growing up, so no treatment is required. In severe hypospadias, the urethra opens at the junction of the penis and scrotum, and even in the perineum. The missing part of the urethra is replaced by a fibrous band, and the penis is short and flat, with obvious or severe downward curvature, and sexual intercourse is impossible in adulthood. At the same time, because the urethra opening is located at the back, ejaculation is outside the vagina, so there is no fertility, and surgical correction is necessary. The operation is usually divided into two stages. The first stage is best performed at around two years old, mainly to cut the cord to straighten the penis; the second stage is performed six months to one year after the first stage, after the local scar tissue softens, mainly to perform urethroplasty. When the penis is grown up, it is difficult to correct the curvature of the penis. Of course, there are also people in the medical community who advocate completing the two stages of the operation in one go, that is, while cutting the penile cord to straighten the curved penis, use a leather tube to make a tube from the bladder mucosa to replace the defective urethra. Most patients with penile induration are middle-aged people. The main symptoms are hyperplasia of penile connective tissue, one or more hard plaques and cords on the back of the penis. When the penis is erect, it bends upward or to the left and right, and feels slightly painful. If the bend is obvious, it affects sexual life. The cause of this disease is not yet clear, and injury and inflammation may be inducing factors. Surgical removal of the nodules is prone to recurrence, so it is not recommended. The general treatment is to inject cortisone locally at the nodules or cords, and take vitamin E orally at the same time. Penile tissue amine or iodine ion penetration can also be performed with the cooperation of the physical therapy department, but in general, the treatment effect is not ideal. Currently, the ideal method is to correct the penile curvature while surgically removing the lesion. Simple penile curvature can occur at any age. It is advisable to perform penile curvature correction surgery as early as possible. 4. Surgical method: The surgical method for penile curvature is to remove the foreskin, inject drugs that make the penis erect or infuse a sufficient amount of physiological saline to observe the erect and curved penis, and then fold the excess cavernous tunica albuginea, and use the tension caused by the tightening of non-absorbable sutures to gradually correct the angle of the penis curvature. In this way, several consecutive folds can completely straighten the entire penis. For most patients, the erect penis cannot lift its head and chest or be parallel to the ground before the operation. After the operation, this is completely changed. The fully erect penis can exceed the parallel line of the ground and even reach the abdomen. This allows the patient to observe whether the correction is satisfactory on the operating table. |
