2. Appendicitis

The most common disease causing right lower abdominal pain in men is appendicitis.

①Acute appendicitis

Typical acute appendicitis has pain in the upper middle abdomen or around the umbilical cord at the beginning, and after a few hours the pain moves and becomes fixed in the right lower abdomen. When the inflammation affects the serous layer and parietal peritoneum, the pain becomes fixed in the right lower abdomen, and the original upper middle abdomen or around the umbilical cord pain is reduced or disappears. Therefore, the absence of a typical history of migratory right lower abdominal pain cannot exclude acute appendicitis.

Simple appendicitis often presents with paroxysmal or persistent bloating and blunt pain. Persistent severe pain often indicates suppurative or gangrenous appendicitis. Persistent severe pain affecting the middle and lower abdomen or both sides of the lower abdomen is often a sign of appendicitis gangrene perforation.

②Chronic appendicitis

Abdominal pain is pain in the right lower abdomen, characterized by intermittent dull pain or bloating, sometimes severe and sometimes mild, and in a relatively fixed location. Most patients experience abdominal pain after a full meal, exercise, fatigue, cold, and long-term standing. Tenderness is the only sign, mainly located in the right lower abdomen, generally with a small range and a constant position, and only occurs when heavy pressure is applied. There is no muscle tension or rebound pain, and generally no abdominal mass.

Patients often experience varying degrees of indigestion and loss of appetite. Patients with a longer course of illness may become emaciated and lose weight. Generally, there is no nausea, vomiting, or abdominal distension, but elderly patients may have constipation.

3. Caused by prostate disease

It has been clinically confirmed that dull pain in the right abdomen caused by prostatitis is very common in clinical practice. Patients should receive targeted treatment according to their condition. If the following conditions occur, it is basically prostate disease.

① Abnormal urination, initially there is discomfort during urination, a feeling of incomplete urination, or a burning sensation, frequent urination, urgency, pain, and itching. A small number of patients find that there is secretion sticking to the urethra in the early morning, and the residual urine after urination soaks the clothes and pants.

② Local or systemic pain, often bloating and cramping, radiating to the glans penis and perineum, can cause pain in the lumbar sacral region, hip, perineum, suprapubic region and abdomen, and sometimes radiating to the testicles and groin.

③ Neurasthenia, manifested by dreaminess, anxiety, restlessness, etc.

④ Sexual dysfunction, manifested as decreased libido, impotence, premature ejaculation, nocturnal emissions, infertility, etc.

4. Ulcer perforation

Sudden severe upper abdominal pain is a typical symptom, and other symptoms include abdominal distension, nausea, vomiting, fever, and even severe peritonitis in severe cases.

5. Acute cholecystitis

The pain usually occurs after meals or in the middle of the night, and is located in the right upper abdomen or upper abdomen. Sometimes the pain spreads to the right shoulder or scapula, and is accompanied by fever, vomiting, jaundice, etc.

6. Acute pancreatitis

Common symptoms are severe and persistent upper abdominal or left upper abdominal pain (sometimes the pain extends to the back), nausea and vomiting, and fever. The pain is slightly relieved when the body bends forward.

7. Intestinal obstruction

Sudden severe upper abdominal pain is a typical symptom, and there are other symptoms such as abdominal distension, nausea, severe vomiting, etc. Common causes include fecal obstruction, intestinal cancer obstruction, intestinal torsion, etc. Severe cases may even cause complications such as septicemia and perforation.
