Oral halitosis

According to statistics, 80% to 90% of bad breath comes from the oral cavity. Untreated caries, residual roots, residual crowns, poor restorations, abnormal anatomical structures, gingivitis, periodontitis and oral mucosal diseases can all cause bad breath. Among them, caries and periodontal diseases are the most common related diseases. Food residues and plaque are often left in deep caries and under the overhanging protrusions of poor restorations. Bacteria ferment and decompose to produce odor. Pulp necrosis or purulent pulpitis can also produce odor without treatment; patients with periodontal disease are often accompanied by a large amount of tartar and plaque. Bacteria ferment in the periodontal pocket to produce hydrogen sulfide, indole and ammonia, thus producing odor. In addition, periodontal abscesses and periodontal pocket pus are mostly caused by Staphylococcus aureus combined with periodontal pathogens, which can also cause bad odor. The quality and quantity of saliva also play an important role. The reduction of saliva volume and the increase of organic components such as protein reduce the flushing and buffering effects of saliva, allowing bacteria to multiply in large numbers, decomposing organic components in saliva, gingival sulcus fluid and food residues, and producing a large amount of volatile sulfides, indoles and other substances, causing bad breath.

Non-oral halitosis

Diseases of the tissues adjacent to the oral cavity, such as purulent tonsillitis, chronic maxillary sinusitis, and atrophic rhinitis, can produce purulent secretions and give off a foul odor; common medical diseases in clinical practice, such as acute and chronic gastritis and peptic ulcers, produce sour and smelly odors; pyloric obstruction and advanced gastric cancer often cause bad breath like rotten duck eggs; patients with diabetic ketoacidosis can exhale acetone-smelling gas, and patients with uremia exhale the odor of rotten apples. In addition, diseases such as leukemia, vitamin deficiency, and heavy metal poisoning can all cause bad breath.

Physiological halitosis

Hunger, taking certain medicines or irritating foods such as onions and garlic, smoking, and reduced saliva secretion during sleep, which causes bacteria to decompose food residues in large quantities, can all cause short-term bad breath. Bad breath in healthy people may be caused by increased and thickened plaque on the back of the tongue due to poor oral habits and oral hygiene. The back of the tongue has a large surface area and many papillae, fissures and depressions, which are conducive to the retention of bacteria, desquamated epithelium of the oral mucosa, food residues, etc., acting as a "bacteria storage room" and facilitating the generation of bad breath. Studies have shown that there is a positive correlation between the degree of bad breath, the amount of volatile sulfides, and the thickness and area of ​​the tongue coating, among which the relationship with the thickness of the tongue coating is closer. Volatile sulfides decrease after removing the tongue coating. This may be because the thicker the tongue coating, the easier it is to form an anaerobic environment, which is more conducive to the growth of anaerobic bacteria, and thus more conducive to the production of volatile sulfides, leading to bad breath.
