Gynecomastia (GYN) is a clinical symptom of abnormal development of male breast tissue and abnormal proliferation of breast connective tissue due to an imbalance in the ratio of estrogen to androgen caused by physiological or pathological factors. GYN is the most common male breast disease, accounting for about 80% to 90% of male breast diseases.

Clinically, it often presents as painless, progressive enlargement of one or both breasts or tender masses in the subareolar area. 50% of GYN is physiological, most common in neonates and adolescents. GYN at this time is often short-lived and usually benign, but if it occurs before puberty, in young people and middle-aged people, it is often considered abnormal and requires further examination to exclude the possibility of breast cancer or other neoplasms.

Gynecomastia is relatively common and can occur in some normal physiological conditions as well as in some diseases. The pathogenesis is complex and may be caused by the following factors:

(I) Relative or absolute increase in estrogen

Testosterone can be converted into estrogen in the body. For particularly sensitive breast tissue, testosterone itself can also cause hyperplasia.

(ii) Increased pituitary gonadotropin

In particular, the luteinizing hormone, also known as prolactin, can cause the production of connective tissue between the mammary ducts and increase adipose tissue.

(iii) Increased sensitivity of breast tissue to the trace amounts of estrogen normally found in male body fluids.

(iv) Use of estrogen or drugs with estrogen-like effects such as reserpine, isoniazid, and human gonadotropin.
