Why do boys want to pee but can't pee?

The inability to urinate is actually called dysuria, which refers to the abnormal phenomenon that the urine accumulated in the bladder cannot be discharged smoothly. The bladder pressure increases due to the accumulation of urine in the bladder, and this consequence is medically called urinary retention.

The process of urination is a complex reflex process. The nerve receptors in the bladder wall sense pressure stimulation, which generates an impulse of excitement that is transmitted to the spinal cord and brain, producing the urge to urinate; the cerebral cortical center issues instructions to contract the bladder detrusor muscle, relax the sphincter, and discharge urine through the urethra. It can be seen that normal urination function depends on the normal function of the nervous system, the coordinated contraction of the bladder muscles, and the patency of the urethra.

Dysuria is very common among the elderly, especially in men. The male urinary system has its own special features: a long urethra and a prostate. This structural feature makes elderly male patients more susceptible to obstructive dysuria. The prostate is a glandular structure surrounding the posterior urethra. Its hypertrophy and hyperplasia can narrow the posterior urethra and make urination difficult.

Prostatic hypertrophy is very common among the elderly. Almost all people over 50 years old can see changes in prostatic hypertrophy, and the incidence and severity increase with age. However, urination difficulties caused by prostatic hypertrophy develop slowly. In the early stage, there is only a drop of urine at the end of urination or each urination is divided into several segments. Then the urine line gradually becomes thinner, the range is shortened, the urination time is prolonged, and the urination is laborious. Finally, the urine can only drip out. The amount of residual urine in the bladder increases for a long time, causing the bladder to lose its contraction ability and urine retention occurs.

In addition to prostate hypertrophy, prostate cancer, urethral stones, blood clots in hematuria, bladder tumors affecting the internal urethral opening, urethritis and benign urethral stenosis caused by physical injury are also causes of obstructive urination. In female patients, uterine fibroids compressing the bladder opening can also cause urination difficulties.

Due to perineal surgery or injury, reflex bladder sphincter spasm may occur, and urine cannot be discharged smoothly.
