Adult scrotal hernia generally refers to a hernia in adults over 18 years old in which the hernia has already protruded and descended into the scrotum. The contents that protrude and descend into the scrotum are mostly the colon or greater omentum, or the sigmoid colon or bladder. Therefore, the symptoms of this type of hernia are mostly a large reducible mass in the groin. The mass appears or increases when standing up, and most of the mass disappears or becomes smaller after lying down. It may be accompanied by discomfort such as pain and soreness. Some people are accompanied by abdominal distension, poor appetite, difficulty in defecation and other main manifestations of gastrointestinal discomfort. In rare cases, there may be sudden acute abdominal pain, vomiting, cessation of defecation and other symptoms of intestinal obstruction.

Hernia is an anatomical abnormality caused by the protrusion of abdominal contents through the weak links of the epidermis. If the hernia falls into the scrotum, it indicates that the hernia is an oblique inguinal hernia. A small number of direct inguinal hernias can fall into the scrotum, but they are extremely rare. Falling into the scrotum indicates that the hernia is very serious and there is a possibility of incarceration of the hernia contents, resulting in the possibility of necrosis. For example, if the hernia contents are the colon, colon necrosis may occur, and if the hernia contents are the retina, retinal necrosis may occur. Therefore, active surgical treatment and hernia repair are required.

Male scrotal hernia refers to inguinal oblique hernia. In severe cases, the hernial sac falls into the scrotum. When the patient stands up, the hernial contents (mostly the intestine) fall into the scrotum, causing scrotal pain and discomfort. If the hernial ring is narrow, there will also be hidden pain symptoms because the herniated intestine stretches, which can cause lower abdominal pain and discomfort.

Most patients can push the hernia contents into the abdomen by themselves, or the hernia contents will return to the abdomen by themselves when lying flat. For patients with narrow hernia ring opening, it may be difficult to reduce or incarcerated, which requires technical professional doctors or emergency surgery.
