What tests should patients with purulent urine undergo?

1. Symptoms associated with purulent urine

Purulent urine accompanied by renal colic indicates that the lesion is in the kidney, which is seen in kidney stones, renal abscesses, polycystic kidney disease, etc.

Symptoms of systemic infection and poisoning suggest upper urinary tract infection.

The medical history should pay attention to low back pain and bladder irritation symptoms: frequent urination, urgency, pain when urinating, and turbid urine.

2. Physical examination of purulent urine

Tenderness and percussion pain in the kidney area, tenderness in the upper ureter (at the flat part of the outer edge of the rectus abdominis), and tenderness in the rib-lumbar point (at the intersection of the psoas major and the twelfth rib) are common in upper urinary tract infection;

When polycystic kidney disease is complicated by infection or upper ureteral obstruction and there is a large amount of renal pelvic effusion, enlarged and deformed kidneys can be felt in the upper abdomen.

3. Laboratory examination of purulent urine

In addition to the increase of white blood cells and pus cells, the routine examination of urine should pay attention to the white blood cell and pus cell casts. The presence of small round epithelial cells indicates urine staining.

The total white blood cell count and neutrophil count in the blood increased.

Urine smears can be used to identify pathogenic bacteria and provide a rapid diagnosis.

Pay attention to kidney function examination in patients with chronic pyelonephritis.

4. Instrumental examination of purulent urine

Ultrasound and CT examinations are necessary for complicated pyelonephritis.

X-ray venous pyelonephrography is helpful in diagnosing chronic pyelonephritis and complicated pyelonephritis.
