1. There is no standard treatment for this disease. Generally speaking, local tumor resection + chemotherapy +/- local radiotherapy is used. As for the treatment effect, due to the low incidence, the number of cases reported by each institution is relatively small, and the results are not entirely consistent. The 4-year survival rate of patients treated with combined chemotherapy and radiotherapy is about 50%-90%. For CD20+ B-cell testicular lymphoma, there have been recent reports abroad that the 5-year long-term survival rate of patients treated with R-CHOP chemotherapy has reached 90%. There are currently no reports on the application of hematopoietic stem cell transplantation in testicular lymphoma.

Testicular lymphoma is prone to metastasis, and common sites of metastasis/recurrence are the contralateral testis, central nervous system, abdominal lymph nodes, skin, etc. Therefore, during chemotherapy, precautionary intrathecal chemotherapy should be performed.

2. Surgical treatment Because the pathology of testicular tumors is extremely complex, there is no consensus on the treatment methods. However, no matter what type of testicular tumor, orchiectomy should be performed first, and further treatment will be determined based on the results of pathological examination. Radiotherapy for testicular cancer clinical stage I and IIa (the maximum diameter of retroperitoneal metastatic lymph nodes is less than 2cm) can be treated with radiation therapy alone, and stage IIb can be treated with radiation and surgery. The efficacy of radiotherapy is similar to that of retroperitoneal lymph node dissection, but it has less damage to sexual function. Combined treatment with radiation and surgery may cause more sexual dysfunction, so currently chemotherapy and surgery are more recommended, rather than combined treatment with radiation and surgery. In clinical stage II, preventive irradiation of the diastolic and supraclavicular regions is not performed.

3. Testicular cancer chemotherapy: Combination chemotherapy with DDP as the main treatment for disseminated testicular germ cell carcinoma has a complete remission rate of 80%. Incomplete remission patients should receive rescue chemotherapy, and 30% of patients can still achieve complete remission; 90% of complete remission patients can survive long-term without cancer. If the serum marker level rises again or the residual mass increases, rescue chemotherapy will be performed.
