Common diseases that can easily cause itching in the male perineum (especially in middle-aged and elderly men) include:

Tinea femoris: Ring-shaped or multi-ring-shaped, occurring at the base of the thigh, sometimes spreading to the buttocks, but not involving the scrotum. The edges are raised, clearly demarcated, and slightly brown. Red papules or blisters may be seen at the edges, with crusts or scales. Fungal examination is positive.

Eczematous epithelial carcinoma: It starts with a scaly erythema, followed by erosion and flushing on the surface, producing exudate, similar to eczema. The edges are distinct, and infiltration gradually occurs. The skin thickens and cracks occur, and there are milky white crusts on the surface, which are itchy.

Carcinoma in situ: more common in the elderly, initially appears as a light pink or red papule covered with a thick crust, gradually developing into a nodule with clear boundaries, the surface showing granular or papillary proliferation and exudate.

Scrotal pruritus: It is a common pruritus in male patients with paroxysmal itching. It is limited to the scrotum at the beginning, but can spread to the perineum and inner thighs over time. There are no skin lesions at the beginning, but after scratching, scabs, pigmentation or exudate may occur. The skin may also become shiny and thickened, showing eczema-like changes.

Flat or shiny lichen planus: It is a kind of erythematous scaly skin disease, which is prone to occur on the genitals and abdomen. It is waxy or hypertrophic and rough lesions, with dense flat papules or millet-sized, shiny, and firm papules.

Pityriasis rosea: It is more common in adults and occurs in spring and autumn. It is seen on the thighs and trunk. The skin damage is irregular oval rose-colored macules, about the size of a broad bean, with a slightly yellow center. The edges are narrow and the surface is covered with bran-like scales.

Scabies: Rare, highly contagious, characterized by red, itchy papules from the scrotum to the glans penis, which can also spread to the whole body. Another characteristic is the itching at night.

Pubic lice: It itches at night, and there are white, beaded eggs in the pubic hair. It is not easy to infect family members.

In addition to the above factors, there are some other reasons that may also cause itching in men's lower body, such as excessive tension in men, or psychological implications after having unclean sex, etc. However, if you want to treat the disease, you must first understand these factors.
