That is, all branches of the internal spermatic vein are ligated at the internal ring (Figure 1). If there is leakage, it often leads to recurrence. After the internal spermatic vein is ligated, the venous blood returning from the testicles, epididymis, etc. can return to the inferior vena cava along the external spermatic vein. Or through the subcutaneous venous branches below the external ring to the femoral vein. Some people suggest ligating the internal spermatic vein at the posterior abdominal wall above the internal ring. The reason is that the vein there has become a branch and will not be missed. The disadvantage is that the location is deep, the exposure is poor, and there is a lot of tissue damage. Once there is an infection, the resistance of the retroperitoneal space is low and it is easy to spread. Therefore, it is not a routine surgical method now. It is only used for recurrence. This surgical approach is taken when there are many adhesions on the original incision.
