Inflammation of the corpus cavernosum

Penile corpus cavernosum inflammation is caused by men not paying attention to their sex life. Penile fibrous cavernosum is also called penile induration. Therefore, it is also called Peyronie's disease. The lesions are mainly confined to the tunica albuginea of ​​the penis and the gap between the tunica albuginea and the erectile tissue. It is characterized by localized fibrous plaques. When the penis is erect, it can form functional shortening and curvature deformity. Patients often have psychological and sexual dysfunction.

Causes

The cause is not well understood. In the 1960s, it was thought to be a perivascular inflammation between the tunica albuginea and the erectile tissue of the penis. However, anti-inflammatory treatments were not effective. It is now believed that penile plaques are scars, not inflammation, but the result of an autoimmune process.

Various endogenous and exogenous factors that regulate collagen synthesis, such as ascorbic acid factor, epidermal growth factor (EGF), insulin-like growth factor (IGF), etc., play an important role in the pathogenesis of penile fibrous cavernosum. As a cytokine, TGF-β affects extracellular matrix deposition and can induce fibrosis of the penile tunica albuginea.

Clinical manifestations

Penile fibrous cavernosum can be divided into acute and chronic stages. The former is characterized by acute inflammatory reactions and pain. The pain mainly occurs when the penis is erect. 30% to 40% of patients have obvious pain symptoms. This stage can last for several months. During this period, nodules may form on the penis and the penis will continue to bend toward the affected side. When the active inflammation of the penis tunica albuginea stops, it enters the chronic stage. During this stage, the pain symptoms usually disappear, the plaques form nodules, and the bent penis cannot recover.

Penile fibrous cavernosum is also known as penile fibromatosis. Physiological processes similar to those of penile fibrous cavernosum occur in some important tissues of the human body, such as Dupuytren's contraction caused by scarring of the palmar fascia; Ledderhose sign caused by scarring of the plantar fascia; and tympanic sclerosis caused by scarring of the tympanic membrane. About 30% of penile fibrous cavernosum is combined with Dupuytren's contraction, while only 3% of Dupuytren's contraction suffers from penile fibrous cavernosum. Although a tendency to familial onset has been noted, no correlation has been found between the disease and human leukocyte antigen (HLA)-B7 or HLA-B27.

Most of the patients are middle-aged, mainly between 45 and 60 years old.

Most cases of fibrous cavernosum of the penis are insidious. Patients have unsatisfactory erectile function, painful plaques on the penis, and slowly progressive penile curvature. There are also cases of sudden onset, with sudden discovery of penile curvature, which generally does not progress.

The deformation of the penis curvature causes difficulty in sexual intercourse. If there is impotence, it is due to psychological disorders or organic changes in the blood vessels of the penis.

Induration or cord-like plaques can be felt on the dorsal side of the penis. They vary in size and may be solitary or multiple. They are tender or mildly tender. They may be painful when the penis is erect. The lesions do not invade the urethra, and there is no urination or ejaculation disorder. When the plaques are calcified or ossified, plain X-rays can show the lesions and measure their size.
