Testicular diseases can easily affect the normal health of men, and can also easily lead to the manifestation of some physical diseases. The appearance of a mass on the testicles is a relatively common disease, which may be caused by varicose veins of the testicles, or a symptom caused by a cyst.

This situation should be a symptom caused by varicocele of the left varicocele. It is recommended to go to the hospital for a thorough examination and receive symptomatic treatment after diagnosis, and surgery if necessary.

Causes

(I) Anatomical factors: The blood of the testicle and epididymis flows back through the spermatic vein, which can be divided into three groups. They have collateral circulations at the external ring to communicate with each other. The rear group: external spermatic vein → inferior epigastric vein → femoral vein → external iliac vein. The middle group: vas deferens vein → superior vesical vein → internal iliac vein. Front group: Internal spermatic vein: The veins of the testis and epididymis mainly return through the spermatic vein plexus. The plexus merges into 2 to 4 veins in the inguinal canal, passes through the internal ring to the retroperitoneum and merges into a vein called the internal spermatic vein. The right internal spermatic vein goes up obliquely into the inferior vena cava; the left one goes into the left renal vein at a right angle.

(ii) Physiological factors: Young men have stronger sexual functions and the blood supply to the scrotal contents is strong. Therefore, some varicocele may gradually disappear with age. In addition, standing for a long time and increasing abdominal pressure are also factors that cause the disease.

(III) Other factors: Retroperitoneal tumors, kidney tumors, hydronephrosis, etc. compress the spermatic vein and can cause symptomatic or secondary varicocele. The primary one disappears quickly when lying down, while the secondary one often does not disappear or disappears very slowly.

Early identification

In view of the fact that varicocele is very harmful to male fertility and the onset age is young, early identification and early prevention are very important. In fact, it is not difficult to detect varicocele. First of all, you should know that this disease is a common disease among boys in the upper grades of elementary school and middle school. Teenagers themselves or their parents can check regularly to see whether the scrotum is at the same height when standing upright and whether earthworm-like venous masses can be seen on the scrotal wall. If venous masses can be seen and one side of the scrotum is lower than the other side, it can be diagnosed as varicocele. If there is no obvious change in the scrotum from the outside, but a venous mass can be felt along the outside of the scrotum; or if the patient can feel the venous mass by holding his breath (increasing abdominal pressure), it is also varicocele. These two situations are relatively mild. If you are unclear and have doubts, you should go to the hospital to ask a specialist for diagnosis.

Varicose veins can be divided into three grades: mild, moderate, and severe. Since it is related to the choice of treatment plan, it should be carefully distinguished. The above-mentioned venous masses can only be felt after holding the breath, which is the lightest, and is defined as Grade I; the venous masses cannot be seen but can be felt, which is the second worst, and is called Grade II; and the venous masses that can be seen with the naked eye are the heaviest, and are defined as Grade III. There are also doctors who use the diameter of the venous masses to distinguish the severity. The specific standard is that venous masses less than 1 cm are defined as Grade I; between 1 cm and 2 cm are defined as Grade II; and those greater than 2 cm are defined as Grade III.

1. The patient may have no symptoms at all and the disease is only discovered during physical examination.

2. Swollen blood vessels like earthworms can be felt or seen in the scrotum, which is called "worms in a bag" in the medical field. The scrotum or testicle on the affected side has a swollen feeling or pain, and the scrotum is enlarged. When standing, the scrotum and testicle on the affected side are lower than the healthy side. Dilated and tortuous veins can be seen on the surface of the scrotum. There is a soft mass like an earthworm when touched. The symptoms are more obvious when standing for a long time or exerting force on the abdomen. Lying flat can reduce or eliminate the symptoms. Sitting for a long time will cause the local temperature of the spermatic cord to rise and cause swelling and pain.

3. Patients may have symptoms of neurasthenia, such as headache, fatigue, neurasthenia, etc. Some patients have sexual dysfunction.

Varicose veins can sometimes affect fertility. 9% of people with varicocele are infertile, and 39% of male infertility is caused by varicocele. Severe cases can cause testicular atrophy.

Clinically, it is found that tall and thin people are more likely to suffer from this disease. Although medicine has not confirmed the relationship between body shape and varicocele, it may be because the varicocele vein forms a right angle when returning to the left renal vein, which affects the return of blood.
