Medication principles

1. Infertility caused by genital tract infection is mainly treated with antibiotics and anti-inflammatory treatments, supplemented by drugs to improve sperm motility.

2. Azoospermia, oligospermia and idiopathic infertility should be treated with sex hormone drugs as the main endocrine therapy

3. For those with low sperm motility, medication to improve sperm motility is the main treatment

4. Patients with infertility caused by varicose veins, vas deferens obstruction, cryptic testis, and epispadias should undergo surgical treatment with endocrine drugs and other auxiliary drugs.

5. Absolutely infertile people (such as azoospermia) should undergo artificial insemination

Treatment includes two aspects: cause treatment and symptomatic treatment:

1. Promote testicular spermatogenesis

① Drug treatment: Commonly used human gonadotropin 1500-2000U intramuscular injection 3 times a week; clomiphene 50mg orally once a day for 3 months

②Surgical treatment: Patients with varicocele and semen abnormalities need high varicocele ligation. Patients with hidden testicles or incomplete testicular descent can undergo testicular descent fixation to promote testicular spermatogenesis.

2. Keep the vas deferens open

Actively treat sexual dysfunction and maintain normal sexual life. For those with congenital vas deferens agenesis, vas deferens obstruction and vasectomy, active surgical treatment should be performed.

3. Improve sperm function

Those with genital tract inflammation should be actively treated with antibiotics; vitamin E, vitamin C and zinc preparations such as complex protein zinc can improve sperm function. For those with immune infertility, the use of condoms can eliminate and reduce the stimulation of sperm antigens on women. Immunosuppressants in large doses for short periods of time and sperm washing and artificial insemination are helpful in treating immune infertility.

4Artificial insemination (AI)

In recent years, in vitro assisted reproductive technology has developed rapidly, including artificial insemination with donor sperm (AID) and artificial insemination with husband's sperm. In particular, intracytoplasmic sperm injection (ICSI) has been used to treat oligospermia, asthenospermia and azoospermia and achieved good results.
