Common bleeding disorders

Thrombocytopenic purpura is a common hemorrhagic disease with complex etiology and difficult treatment. In severe cases, there may be bleeding in other parts of the body, such as nose bleeding, gum oozing, excessive menstruation or severe vomiting blood, hemoptysis, blood in the stool, hematuria and other symptoms. Intracranial hemorrhage is the cause of death of this disease. Allergic purpura is characterized by scattered spots on the skin of the limbs or affecting the whole body. Severe patients are accompanied by joint pain or abdominal pain, blood in the stool, vomiting blood, and metrorrhagia. Severe cases may develop into purpuric nephritis. Thrombocytopenic purpura is divided into primary and secondary types. Idiopathic thrombocytopenic purpura is an immune syndrome and a common bleeding disorder.

Clinically divided into chronic and acute

Clinically, it can be divided into acute and chronic types, and the pathogenesis and manifestations of the two are significantly different. The acute type is mostly children under 10 years old, with no difference between the two sexes. It often occurs in winter and spring, and there is a history of viral infection before the onset of the disease, mostly upper respiratory tract infection, rubella, measles, and chickenpox; it can also occur after vaccination, and the incubation period between infection and purpura is mostly within 1 to 3 weeks. The acute type in adults is rare, often related to drugs, and the condition is more serious than that in children. The onset is rapid and may be accompanied by fever. It is mainly skin and mucosal bleeding, which is often more severe. Skin bleeding is in the form of petechiae of varying sizes, unevenly distributed, and mostly on the limbs. Mucosal bleeding includes epistaxis, gingival bleeding, and oral and tongue mucosal blood blisters. There is often gastrointestinal and urinary tract bleeding, subsynechiaal bleeding, and a small number of retinal bleeding. Spinal or intracranial hemorrhage is common and can cause paralysis of the lower limbs or manifestations of intracranial hypertension, which can be life-threatening. The chronic type is more common, accounting for 80% of primary thrombocytopenic purpura, mostly in patients aged 10 to 40 years old, and women are 3 to 4 times more likely to be than men. The onset is insidious. Patients may have persistent bleeding or recurrent attacks, and some show local bleeding tendency, such as recurrent epistaxis or excessive menstruation. Petechiae and ecchymoses can occur on the skin and mucous membranes of any part, but are more common in the distal extremities. There may be bleeding in the digestive tract and urinary tract. Deep hematomas may also occur after trauma. Intracranial hemorrhage is rare, but it can still occur during acute attacks. The spleen can occasionally be touched during a deep inhalation.
