1. Urogenital tract diseases

Among patients with urinary system symptoms, 27% have abnormal prostate fluid test results, or abnormal semen bacteriology or biochemical examinations, indicating that some of them suffer from adrenal gland inflammation.

In addition, in the whole population, the incidence of azoospermia is similar between those with and without sexually transmitted diseases, but the incidence of abnormal semen analysis is significantly higher in the former than in the latter. Sexually transmitted diseases do not seem to affect sperm density, indicating that they do not cause stenosis or blockage of the vas deferens. Patients with sexually transmitted diseases generally show decreased sperm motility, which is related to disorders of accessory gland function, especially epididymitis.

2. Testicular Disease

There are few people with a history of testicular torsion, but it is often complicated by azoospermia and oligospermia.

Patients with orchitis and epididymitis caused by non-mumps have significantly reduced sperm density and motility. These patients also have smaller testicles and a higher incidence of epididymitis.

The incidence of azoospermia is significantly increased in men with a history of poor testicular descent (i.e., testicles that do not descend normally into the scrotum). The incidence of azoospermia in men with bilateral poor testicular descent is twice as high as that in men with unilateral testicular descent. Even if these patients have sperm, the semen quality is abnormal, especially the oligospermia rate is significantly increased.

Testicular damage. Only those infertile patients with scrotal hematoma or hematuria, indicating testicular or genitourinary tract damage, are recorded here. The incidence of azoospermia or semen abnormalities is significantly increased in such patients.

3. Other systemic diseases

Bronchitis, especially bronchiectasis, is often a clinical manifestation of systemic ciliary dysfunction. The incidence of azoospermia is significantly increased in such patients. The testicular texture, volume and biopsy findings of all such patients are normal, indicating that azoospermia is caused by obstruction of the vas deferens. The abnormal rate of semen analysis in patients with bronchitis increases, and the average sperm density and the proportion of sperm moving forward are lower.
