To treat acute epididymitis, first we need to understand what it is and what are the symptoms: This disease often develops rapidly, with symptoms such as general discomfort, high fever and increased white blood cell count. It often occurs on one side and rarely on both sides. The scrotum is swollen and the pain is severe, which can radiate to the ipsilateral abdominal groove area and lower abdomen. The scrotum is enlarged and the skin is red. Examination shows that the epididymis is enlarged and obviously tender. In the early stage, it has a clear boundary with the testicle, and the seminal vesicle is edematous and thickened.

1. Treatment

① General treatment: Rest in bed. Using a scrotal support can alleviate symptoms. A homemade larger scrotal support with cotton pads will be more comfortable to use. Painkillers can be used for severe pain. Local heat therapy can relieve symptoms and promote the disappearance of inflammation. However, using heat therapy too early can aggravate pain and has the risk of promoting the spread of infection, so it is advisable to use ice packs for local cold compresses in the early stage. Sexual intercourse and physical labor can aggravate infection, so they should be avoided.

②Application of antibacterial drugs: Choose drugs that are sensitive to bacteria. Usually, intravenous administration for 1-2 weeks is followed by oral antibiotics for 2-4 weeks to prevent the disease from turning into chronic inflammation.

③Surgical treatment: If antibiotic treatment is ineffective and testicular ischemia is suspected, epididymis incision should be performed to relieve pressure, and the epididymis lining should be incised longitudinally or horizontally, but the epididymis duct should be avoided.

2. Prognosis

Acute epididymitis usually recovers without complications if it is diagnosed promptly and treated appropriately. It takes about 2 weeks for symptoms to disappear completely, and 4 weeks or longer for the epididymis to return to normal size and texture. Bilateral acute epididymitis can lead to decreased fertility or infertility in patients, but this is rare in clinical practice.

Epididymitis is a common men's disease and is also the necessary path for sperm. If this disease is not treated, it will cause male sperm and kidney deficiency, lead to infertility, and reduce male sexual function, etc. In severe cases, it may even threaten a man's life.
