(1) Male urethritis can cause infertility

Urethritis is divided into two categories: gonococcal urethritis and non-gonococcal urethritis. An expert in male disease research said that gonococcal urethritis is abbreviated as gonorrhea, which is mainly transmitted through sexual intercourse, and the pathogen is Neisseria gonorrhoeae. The main manifestation of gonorrhea in men is acute urethritis, which can invade the prostate, epididymis and bulbourethral glands, and can spread through the blood to cause arthritis, endocarditis and even meningitis, and can also spread to the peritoneum to cause perihepatitis. After infection with gonococci, the incubation period is generally 2 to 7 days. The earliest symptom is yellow purulent secretions in the urethra, accompanied by burning pain and itching in the urethra during urination, and redness, swelling and eversion of the urethral opening. If gonococci are found in the secretions, the diagnosis can be confirmed. If it is not certain that it is gonorrhea, bacterial culture can be performed.

Non-gonococcal urethritis is mostly caused by Chlamydia trachomatis, but in a few cases it can also be caused by ureaplasma urealyticum. In patients with Chlamydia trachomatis infection, inclusion bodies can be found in the urethral secretions, and tissue culture is positive. The incubation period is generally 2 to 3 weeks, and can even be as long as 5 weeks. The symptoms of urinary pain are mild, the urine secretions are white and mucous, there are fewer white blood cells in the secretions, and gonococci cannot be found. Experts in andrology believe that urethritis can cause male infertility. The main reason is that inflammation causes urethral stenosis and obstruction of the urethral opening. If it affects the vas deferens and epididymis, it can cause fibrous tissue hyperplasia, deformation, and scar formation, resulting in obstruction of the vas deferens. When it is complicated by inflammation of the epididymis, seminal vesicles, and prostate, it can affect the quality of semen and reduce sperm survival rate and motility.

(2) Be careful of urethritis causing arthritis

Urethritis can induce aseptic arthritis, which is related to the presence of cold-prone genes in the patient's body. Reactive arthritis is aseptic arthritis induced after urethritis and cervicitis, such as reactive arthritis after non-gonococcal urethritis and reactive arthritis after bacterial diarrhea. In addition, mycoplasma, spirochetes, Brucella, Chlamydia pneumoniae, etc. can also induce reactive arthritis. Experts in andrology said that the characteristic of reactive arthritis is that it usually occurs in the large joints of the patient's hands and feet, and the small joints are generally not affected, and it does not occur symmetrically. There is currently no effective prevention method for this disease, but the prognosis of this disease is very good, and there are no dietary taboos. Once the disease occurs, the patient should go to a regular hospital for treatment. Reactive arthritis should be differentiated from other arthritis such as gout, osteoarthritis, and rheumatoid arthritis. Gout lasts for a shorter time, with severe pain that reaches a peak in a short time, while reactive arthritis lasts for a longer time, up to several months; and gout usually has a slow erythrocyte sedimentation rate, but high uric acid, while reactive arthritis has a recent history of infection, a fast erythrocyte sedimentation rate, and often low uric acid. Osteoarthritis often has a history of fatigue, low uric acid, and rheumatoid arthritis often has multiple small joint swellings.

In the face of the possible harm caused by these diseases, what good methods can we use to prevent male urethritis?

(1) Instruct the patient to avoid sexual activity before recovery; abstain from alcohol, spicy food, and drink plenty of water.

(2) Proper isolation should be implemented at home. Bath towels, bathtubs, washbasins, toilets, etc. should be used separately or disinfected after use.

(3) Tell patients what safe behavior is, what dangerous behavior is, and how to avoid dangerous behavior.

(4) Encourage the use of condoms.

(5) Drink more water and eat more liquid food, and urinate regularly, preferably every 3 hours, to help clean the urethra;

(6) The urethra area must be kept clean, especially the urethral opening must be cleaned promptly after urination to avoid bacterial infection of the urethra.

(7) Urinate as soon as possible after sexual intercourse to flush away bacteria and reduce accumulation.

In addition: Holding urine for a long time will cause more and more urine to accumulate in the bladder, and the urine containing bacteria and toxic substances cannot be discharged in time. At the same time, holding urine causes bladder distension, compression of blood vessels in the bladder wall, ischemia of the bladder mucosa, and reduced resistance. Therefore, bacteria will take advantage of the opportunity to enter, easily causing cystitis, urethritis, urinary pain, hematuria or inuria.
