Local symptoms: Early prostate cancer may not show symptoms, but the occurrence of symptoms is related to the gradual enlargement of the tumor and bladder neck obstruction. More than 75% of prostate cancer patients may experience urinary tract obstruction symptoms, such as dysuria, thin urine flow, slow urine flow, short urine flow range or nocturia, etc. Along with urinary tract obstruction, urinary tract infection and prostate infection may also occur, manifested as frequent urination and urgency.

Metastatic symptoms: About 1/20 of prostate cancer patients are not diagnosed until they have metastatic symptoms. Common metastatic symptoms are lumbosacral pain that radiates to the hip and leg. A mass in the right upper abdomen indicates liver metastasis; a mass above the clavicle indicates lymph node metastasis; unexplained bone pain indicates metastasis to the bone in that area. In addition, anemia and weight loss may also occur. Hematuria may also occur in the late stage of the disease, due to urinary tract obstruction and renal dysfunction caused by the tumor. Bone metastasis is particularly important in diagnosis.

Prostate cancer has the characteristics of early metastasis, mainly lymphatic and hematogenous metastasis. If it metastasizes to the pelvic bones and pelvic lymph nodes, it is easy to metastasize to the lungs, liver, brain, skin, etc. In addition, organs near the prostate, such as the bladder, sacrum, and seminal vesicles, will be directly affected by the spread and metastasis.

Pathological observation shows that prostate cancer is yellow-white, very hard, and of varying sizes. Under the microscope, a large amount of prostate tissue is seen to be epithelialized or hard cancerous, and the epithelial component shows papillary characteristics. Prostate cancer is usually divided into four stages:

Stage A: Rectal examination is normal and the tumor is discovered incidentally during treatment for benign prostatic hyperplasia or at autopsy.

Stage B: The prostate lump is confined to the capsule and serum acid phosphatase is normal.

Stage C: The tumor infiltrates outside the capsule and serum acid phosphatase is elevated.

Stage D: There is distant metastasis.
