1. Epididymal tuberculosis

The symptoms are epididymal nodules and pain. Most patients have a history of urinary tuberculosis, and their vas deferens are thickened and hardened, with a beaded appearance. Epididymal nodules are mostly located at the tail, hard and irregular, and sometimes adhere to the scrotal skin, rupture and form purulent sinuses. Microscopic examination of secretions can find acid-fast bacteria.

2. Spermatocele

It also manifests as nodules in the epididymis, but the nodules are mostly located in the epididymal head, with a smooth surface and no tenderness. B-ultrasound shows a cystic mass in the epididymal head.

3. Scrotal filariasis

The symptoms are epididymal nodules and scrotal pain. However, the patient has a history of filarial infection, and there are often several nodules in the scrotum, mostly at the lower end of the spermatic cord and the epididymal head. Microfilariae can be found in blood samples taken at night.

4. Epididymal tumor

It also presents as an epididymal mass, and sometimes scrotal swelling and pain. However, the mass is mostly located at the tail of the epididymis, with an uneven surface, unclear boundaries, and a hard texture. Surgical pathological histological examination can confirm the diagnosis.
