1. Hematuria

85% of patients may experience recurrent painless intermittent gross hematuria. The amount of bleeding may be more or less, with blood clots in severe cases. 100% of patients will experience hematuria sooner or later during the course of bladder cancer.

2. Symptoms of bladder irritation

Necrosis, infection, and blood clots can all be stimuli that cause the bladder muscles to contract and produce the urge to urinate; frequent urination, urgency, pain, and persistent urge to urinate, persistent lumbar distension and pain, and urinary incontinence when cancer invades the sphincter. Bladder irritation symptoms are generally a sign of a poor prognosis.

3. Difficulty urinating

Tissue shedding, the tumor itself, or blood clots block the internal opening of the bladder, causing dysuria in about 7% of cases, or even urinary retention.

4. Symptoms of upper urinary tract obstruction

When the ureteral orifice is invaded, the renal pelvis and ureteral orifice will be dilated and accumulate water, or even infected, causing varying degrees of backache, back pain, fever, etc. If bilateral ureteral orifices are invaded, symptoms of acute renal failure may occur.

5. Lower abdominal mass

About 3% of patients present with this as the first symptom, mostly at the top of the bladder or other parts. Rectal (or vaginal) digital examination or dissection of the uneven hard mass is used to understand the scope and depth of the tumor infiltrating the bladder wall, which is helpful for the staging of the cyst.

6. Systemic symptoms

Nausea, loss of appetite, fever, weight loss, anemia, weakness, cachexia, leukemoid reaction, etc.

Some people may experience some symptoms such as difficulty in defecation and stinging when urinating, but many people with bladder cysts will have hematuria to a certain extent.
