The onset of urinary incontinence in patients with prostatic hyperplasia is gradual and is not noticed by the elderly at first. The initial symptom is that when the urge to urinate is present, the urine cannot be discharged immediately, and it often takes several seconds or even minutes to discharge the urine. We call this condition hesitant urination.

The hyperplastic prostate tissue causes the posterior urethra to enlarge, bend and be compressed, or the hyperplastic prostate protrudes to the bladder neck to produce a spherical valve, which can increase the friction resistance of urination. The obstruction of the lower urethra is not only related to the size of the hyperplastic gland duct, but also to the location of the hyperplasia and whether the prostate tissue grows mainly inward or outward.

Patients with mild prostatic hyperplasia have slow urination, weak urine flow, short effective range, narrowed or bifurcated urine flow. Bifurcated urine flow is mostly caused by mid-stage hyperplasia that pushes up the middle part of the urethra, causing urine to be discharged from both sides. In severe cases of prostatic hyperplasia, the patient needs to hold his breath several times when urinating, and the waist muscles are also contracted to increase abdominal pressure to help urination.

When the patient holds his breath for a long time and needs to take a deep breath to urinate, the patient's urine stream will be interrupted as the abdomen lowers, and intermittent urination will occur. When the condition develops to a more serious stage, the patient's urine stream can no longer form a straight line, but only drips and takes a long time to discharge, or even completely fails to discharge urine.
