There are many reasons for pathological testicular shrinkage, the most common of which are chronic wasting diseases, endocrine disorders (hypothyroidism and hypopituitarism), persistent high fever, taking large amounts of female hormones, viral orchitis (such as mumps complicated with orchitis), testicular syphilis, etc. These diseases can cause testicular atrophy, resulting in a volume smaller than normal.

Pathological testicular atrophy and reduction in size is usually caused by the involvement of spermatogonia or spermatocytes in the seminiferous tubules. When these cells atrophy and cannot produce sperm, or are damaged and cannot produce normal sperm, the testicles atrophy and decrease in size.

In addition, it is often accompanied by infertility; however, because the interstitial cells that secrete male hormones are less affected, it generally does not affect the development of male reproductive organs and male secondary characteristics (such as beard growth, lower voice, prominent Adam's apple, muscular development, etc.). From a clinical perspective, severe testicular atrophy leading to infertility is rare.

Whether testicular atrophy is accompanied by fertility disorders can be examined by semen microscopy. If the sperm count per milliliter of semen is less than 20 to 60 million, and the activity is reduced and the morphology is abnormal, infertility should be considered.

First, you should pay attention to the size of the testicles. Testicle size is related to age. For children under 12 years old, the testicles are in a relatively quiescent period, and the general volume is 1 to 4 ml. After 12 years old, boys enter puberty and the testicle volume increases rapidly. The testicle volume of adult men is 16 to 25 ml, with an average of 19.8 ml. After the age of 60, they enter the aging stage, and the testicle volume gradually shrinks. After the age of 70, the testicle volume shrinks to less than 16 ml. If the testicle volume of both sides of an adult male is less than 10 ml, it is a small testicle, which may cause sexual dysfunction or male infertility.

Palpate and examine the testicles. The normal state of the testicles should be smooth, full, non-tender, and clearly separated from the epididymis. If the testicles are found to be enlarged in a short period of time and are not tender, it may be testicular cancer; if the testicles are painful when touched, it indicates that the testicles are inflamed; if the scrotum is enlarged and the testicles and epididymis cannot be felt, it may be hydrocele, that is, fluid accumulation in the scrotum; if the epididymis is painful and swollen, it indicates that the epididymis has a lesion

Through the above introduction, we can understand that there are many reasons that lead to small testicles, but it will not have any effect on the fertility of male friends. If you are not very assured, you can go to a regular hospital to check your fertility. This is safer and more reliable.
