The appearance of a lump at the base of the male penis is a common physical condition in medicine. The appearance of a penis lump may be caused by a disease or a penile nodule. It is mainly a lump in the penis area of ​​the male reproductive organ, which can easily cause pain. It can be treated by surgery. Pay more attention to observation at ordinary times.

What is the hard lump at the base of the penis?

There is an obvious lump at the bottom of the male penis, which is called penile induration. Penile induration is also one of the many causes of penile deformity in clinical practice. Penile induration is also called fibrous cavernosum, or Peyronie's disease.

Clinical manifestations are one or two nodules on the penis body, which may lead to penile curvature. Generally, penile curvature can only be manifested when the penis is erect, and the nodules themselves may be painless. Penile sclerosis is a plaque-like or cord-like nodule formed by the proliferation of connective tissue between the tunica albuginea of ​​the corpus cavernosum and the fascia of the penis. It is not adhered to the penile skin and does not involve the corpus cavernosum and urethra.

The principle of treating penile induration is the same, that is, through surgical and non-surgical treatment. Anti-inflammation, detumescence and anesthesia are used to soften the patient's induration and eventually disappear. The effects of these treatments vary greatly, but even so, patients should actively cooperate with doctors for treatment.

Penile induration refers to the fibrotic changes of the tunica albuginea of ​​the corpus cavernosum of the penis, which causes single or multiple plaques or nodules to appear on the back or outside of the penis. Therefore, it is also called Peyronie's disease. This disease is more common in adults, because the plaques can cause erectile pain and penile curvature deformity, causing difficulties in sexual life. However, this disease develops slowly and has no tendency to become malignant.

Causes

The etiology of this disease is still unclear, but it is suspected to be related to chronic injury and inflammation of the penis, vitamin E deficiency, genetic factors, sexually transmitted diseases, and autoimmune reactions.

Clinical manifestations

The severity of Peyronie's disease can be mild, moderate or severe, and symptoms can develop gradually or overnight. The main clinical symptoms include the following:

1. Local induration

Painless hard plaques appear on the penis. The nodules are mostly located on the back of the penis. Small ones are the size of rice grains, while large ones can spread to the entire back of the penis. They are round, cord-like or plaque-like, hard in texture, fixed and immobile, and the skin and subcutaneous tissue are usually not affected.

2. Penis curvature

Most patients only experience penile curvature during erection. A lump on the upper side of the penis will cause the penis to bend upward, while a lump on the lower side of the penis will cause the penis to bend downward. The curvature is usually 30°-120°.

3. Shortening and deformation of the penis

Some patients may have lumps on both the upper and lower sides of the penis, shortening and deforming the penis.

4. Erection pain

The penis feels uncomfortable or painful when it is erect. There is usually no pain when it is not erect, and some people only feel pain during sexual intercourse.

Due to pain and curvature during erection, some patients have erectile dysfunction and difficulty or failure in sexual intercourse due to huge plaques affecting the blood supply to the corpus cavernosum.

treatment

1. Drug treatment

(1) Local injection of collagenase is effective for mild symptoms, and local injection of verapamil into the plaque may also be effective.

(2) Oral administration of large doses of vitamins, along with oral administration of diethylstilbestrol and potassium iodide can be used as adjuvant therapy.

(3) Anti-fibrotic drugs, such as potassium aminobenzoate.

(4) Drugs that inhibit connective tissue proliferation, such as hydrocortisone acetate, dexamethasone, procaine, etc., are effective for early lesions.

2. Physical therapy

Diathermy or ultraviolet radiation can be used as adjunctive therapy to improve symptoms.

3. Surgical treatment

The purpose of surgical treatment is to prevent the penis from bending during erection and remove the nodules. Currently, the commonly used surgical methods include surgical nodule removal and prosthesis implantation, but the surgical effect is not very ideal and the disease is prone to recurrence after surgery.

Prognosis

Drug treatment only delays the course of the disease, and surgical treatment is not ideal.
